Overview of the medicine
Bisoprolol is a cardioselective beta-1 adrenergic blocking agent, primarily used to treat high blood pressure, angina, and chronic stable heart failure.
Uses & Indications
Dosage
Adults
Hypertension/Angina: Initially 5 mg once daily, adjusted to 10 mg once daily if needed. Maximum 20 mg once daily. Chronic heart failure: Individualized, initial dose 1.25 mg once daily, titrated gradually up to a maximum of 10 mg once daily.
Elderly
No specific dose adjustment required unless there is severe renal or hepatic impairment. Start with the lowest effective dose.
Renal_impairment
For severe renal impairment (creatinine clearance < 20 ml/min), the initial dose should be 2.5 mg daily, and caution should be exercised. Maximum 10 mg daily.
How to Take
Take orally, once daily in the morning, with or without food. Swallow the tablet whole with some liquid, do not chew or crush.
Mechanism of Action
Bisoprolol selectively blocks beta-1 adrenergic receptors, primarily in the heart, leading to a decrease in heart rate, myocardial contractility, and cardiac output, thereby reducing blood pressure and myocardial oxygen demand.

Excretion
Approximately 50% excreted unchanged in urine, and the remaining 50% as inactive metabolites via urine and bile.

Absorption
Well absorbed from the gastrointestinal tract, with an oral bioavailability of approximately 80-90%. Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Approximately 50% metabolized in the liver to inactive metabolites. The remaining 50% is excreted unchanged.
Side Effects
Contraindications
- •Acute heart failure or decompensated heart failure requiring intravenous inotropic therapy
- •Cardiogenic shock
- •Second or third degree AV block (without a pacemaker)
- •Sick sinus syndrome (without a pacemaker)
- •Severe bradycardia (heart rate < 60 bpm before treatment)
- •Severe hypotension (systolic blood pressure < 100 mmHg)
- •Severe bronchial asthma or severe chronic obstructive pulmonary disease (COPD)
- •Severe forms of peripheral arterial occlusive disease or Raynaud's syndrome
- •Metabolic acidosis
- •Hypersensitivity to bisoprolol or any component of the formulation
Drug Interactions
Clonidine
Increased risk of rebound hypertension if clonidine is withdrawn abruptly while bisoprolol is continued.
Insulin and oral antidiabetics
Beta-blockers may mask symptoms of hypoglycemia (e.g., tachycardia).
Digitalis glycosides (e.g., digoxin)
Increased risk of bradycardia and AV block.
NSAIDs (Nonsteroidal anti-inflammatory drugs)
May reduce the hypotensive effect of bisoprolol.
Calcium channel blockers (e.g., verapamil, diltiazem)
Increased risk of hypotension, bradycardia, and AV conduction disturbances.
Other antihypertensives (e.g., ACE inhibitors, diuretics)
Enhanced hypotensive effect.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency. Treatment is supportive and symptomatic, including intravenous atropine for bradycardia, glucagon, and vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category C. Bisoprolol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, caution advised during lactation.
