Description
Overview of the medicine
Budefort 100 mcg Inhalation Capsule is a combination medicine containing Budesonide, an inhaled corticosteroid, and Formoterol, a long-acting beta2-agonist. It is used for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
Asthma: 1-2 capsules (Budesonide 100 mcg + Formoterol 6 mcg) twice daily. COPD: 1-2 capsules (Budesonide 100 mcg + Formoterol 6 mcg) twice daily.
Elderly
No specific dose adjustment is required, but monitor for side effects.
Renal_impairment
No specific dose adjustment is required for renal impairment.
How to Take
For oral inhalation only. The capsule should be placed in a dry powder inhaler device (e.g., Novolizer, Revolizer) and the contents inhaled. Do NOT swallow the capsule.
Mechanism of Action
Budesonide is a corticosteroid that reduces inflammation in the airways. Formoterol is a long-acting beta2-agonist that relaxes the smooth muscles of the airways, leading to bronchodilation. Together, they improve lung function and reduce symptoms of asthma and COPD.
Pharmacokinetics
Onset
Bronchodilation within 1-3 minutes (Formoterol).
Excretion
Mainly excreted in urine and feces as metabolites.
Half life
Budesonide: 2-3 hours; Formoterol: 10 hours.
Absorption
Budesonide is rapidly absorbed from the lungs, with systemic bioavailability of about 39%. Formoterol is also rapidly absorbed from the lungs, with systemic bioavailability of about 48%.
Metabolism
Budesonide: extensively metabolized in the liver via CYP3A4 to inactive metabolites. Formoterol: extensively metabolized primarily by direct glucuronidation and O-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to budesonide, formoterol, or any excipients.
- •Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- •Not for acute bronchodilator therapy.
Drug Interactions
Beta-blockers
May block the bronchodilatory effect of formoterol and cause severe bronchospasm in asthma patients.
Diuretics, Xanthine derivatives, Steroids
May potentiate the hypokalemic effect of formoterol.
Ketoconazole, Itraconazole, Ritonavir (CYP3A4 inhibitors)
May increase systemic exposure to budesonide, leading to increased risk of systemic corticosteroid effects.
Tricyclic antidepressants, Monoamine oxidase inhibitors (MAOIs)
May potentiate the effect of formoterol on the cardiovascular system.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to excessive beta-adrenergic stimulation (e.g., tremor, palpitations, tachycardia, headache, hypokalemia, hyperglycemia) and/or corticosteroid effects (e.g., Cushingoid features, adrenal suppression). Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Budesonide is excreted in breast milk; however, formoterol is not known to be. Consult a doctor before use during pregnancy or lactation.
