Description
Overview of the medicine
Budefort 200 mcg Inhalation Capsule is a combination medicine containing Budesonide (an inhaled corticosteroid) and Formoterol (a long-acting beta2-agonist), used for the regular treatment of asthma and chronic obstructive pulmonary disease (COPD) in adults and adolescents.
Uses & Indications
Dosage
Adults
Asthma: 1-2 inhalations twice daily. COPD: 2 inhalations twice daily. The dose should be titrated to the lowest effective dose.
Elderly
No dose adjustment is generally required, but monitor for side effects.
Renal_impairment
No specific dose adjustment is recommended for patients with renal impairment.
How to Take
For inhalation use only. The capsule should be placed in a suitable dry powder inhaler device (e.g., Rotahaler, Revolizer). Inhale the powder through the mouth. Do not swallow the capsule.
Mechanism of Action
Budesonide acts as an anti-inflammatory agent in the airways, reducing swelling and irritation. Formoterol acts as a bronchodilator by relaxing the smooth muscles of the airways, leading to their widening and easier breathing.
Pharmacokinetics
Onset
Bronchodilation begins within 1-3 minutes (due to Formoterol).
Excretion
Budesonide is primarily excreted via urine and feces as metabolites. Formoterol is mainly excreted via urine as metabolites and unchanged drug.
Half life
Budesonide: approximately 2-3 hours; Formoterol: approximately 10 hours.
Absorption
Budesonide has high systemic bioavailability (~39%) after inhalation, while Formoterol is rapidly absorbed with peak plasma concentrations within 10-30 minutes.
Metabolism
Both Budesonide and Formoterol are primarily metabolized in the liver by CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Budesonide, Formoterol, or any excipients.
- •Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- •Not for children under 6 years of age.
Drug Interactions
Other sympathomimetics
May potentiate adverse effects of Formoterol.
Diuretics (thiazide, loop)
May potentiate hypokalemia (risk associated with Formoterol).
Beta-blockers (including eye drops)
May reduce the effect of Formoterol and cause bronchospasm.
Ketoconazole, Itraconazole, Ritonavir
May increase systemic exposure to Budesonide, increasing the risk of corticosteroid side effects.
Tricyclic antidepressants, Monoamine Oxidase Inhibitors (MAOIs)
May increase the risk of cardiac arrhythmias with Formoterol.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep capsules in the blister strip until use. Do not refrigerate or freeze. Keep out of reach of children.
Overdose
Symptoms of Formoterol overdose may include tremor, headache, palpitations, tachycardia, metabolic acidosis, hypokalemia, and hyperglycemia. Budesonide overdose typically leads to temporary adrenal suppression. Treatment is symptomatic and supportive; hospitalization may be required in severe cases.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Budesonide is generally considered safe in pregnancy, but consult a doctor. Use with caution during lactation; small amounts of budesonide can pass into breast milk.
