Budefort-400 mcg Inhalation Capsule is a combination medicine containing Budesonide (an inhaled corticosteroid) and Formoterol Fumarate Dihydrate (a long-acting beta2-agonist). It is used as a maintenance treatment for asthma and chronic obstructive pulmonary disease (COPD) to improve breathing and reduce flare-ups.

Dosage
Adults
Asthma: Usually 1 inhalation (400 mcg Budesonide/12 mcg Formoterol) once or twice daily. COPD: 1 inhalation (400 mcg Budesonide/12 mcg Formoterol) twice daily.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment.
How to Take
This medicine is for inhalation use only. It should be inhaled using a dry powder inhaler device (e.g., Rotahaler, Revolizer) as directed by your doctor. The capsule should not be swallowed. Always rinse your mouth with water after inhalation to prevent oral thrush.
Mechanism of Action
Budesonide is a potent glucocorticoid that exerts anti-inflammatory effects in the lungs, reducing airway inflammation and hyperresponsiveness. Formoterol is a selective long-acting beta2-adrenergic agonist that relaxes the smooth muscles of the airways, leading to bronchodilation and improved airflow. Side Effects
Contraindications

Drug Interactions
Beta-adrenergic blockers
May diminish the effect of formoterol. Non-cardioselective beta-blockers should generally be avoided.
Diuretics (e.g., furosemide, hydrochlorothiazide)
May potentiate hypokalemia.
Ketoconazole, Itraconazole, Ritonavir (CYP3A4 inhibitors)
May increase systemic exposure to budesonide, increasing the risk of corticosteroid side effects.
Tricyclic antidepressants, Monoamine oxidase inhibitors (MAOIs)
May prolong QTc interval and increase the risk of ventricular arrhythmias with formoterol.

Overdose
Symptoms of overdose for budesonide are typically limited to systemic corticosteroid effects. Formoterol overdose may lead to exaggerated beta2-agonist effects such as tremor, headache, palpitations, and tachycardia. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Budesonide is excreted in human milk. Formoterol has not been detected in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
