Description
Overview of the medicine
C-3 100 mg Suspension is a broad-spectrum oral antibiotic containing Cefixime, used to treat a variety of bacterial infections, especially in children. It belongs to the cephalosporin class of antibiotics.
Uses & Indications
Dosage
Adults
Typically not used in suspension form for adults. For adults and children > 12 years or > 50 kg: 400 mg once daily, or 200 mg every 12 hours.
Elderly
No specific dose adjustment for elderly with normal renal function; dosage adjustment is required for renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose 200 mg daily. For creatinine clearance < 20 mL/min, dose 100 mg daily.
How to Take
Administer orally, with or without food. Shake the bottle well before each use to ensure proper mixing of the suspension. Use a calibrated measuring device (spoon or syringe) for accurate dosing, especially for children.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located in the bacterial cell wall, thereby interfering with the transpeptidation step of peptidoglycan synthesis. This leads to bacterial cell lysis and death.
Pharmacokinetics
Onset
Varies based on infection type and severity, generally within hours to days of initiating therapy.
Excretion
Primarily renal (about 50% unchanged drug) and some biliary excretion.
Half life
3-4 hours.
Absorption
Approximately 30-50% absorbed orally, with or without food. Peak plasma concentrations are achieved in 2-6 hours.
Metabolism
Not extensively metabolized in the liver; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin (due to potential for cross-reactivity).
Drug Interactions
Probenecid
Increases cefixime plasma levels and prolongs its half-life by decreasing renal tubular secretion.
Carbamazepine
Concomitant administration may result in increased carbamazepine plasma concentrations.
Warfarin and other anticoagulants
May increase anticoagulant effect, leading to prolonged prothrombin time (INR). Close monitoring of INR is recommended.
Storage
Store the unreconstituted powder at room temperature (below 30°C) in a dry place, away from direct sunlight and moisture. Once reconstituted, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the recommended period (e.g., 7 or 14 days). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote for Cefixime. Symptomatic and supportive treatment is recommended. Hemodialysis and peritoneal dialysis are not effective in removing Cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Caution should be exercised when Cefixime is administered to a nursing mother, as small amounts may be excreted in breast milk.

YesClinical Trials
Extensive clinical trials have been conducted worldwide to establish the efficacy and safety of Cefixime in treating various bacterial infections in both pediatric and adult populations. These trials support its use for the listed indications.
Lab Monitoring
- Renal function tests (for prolonged therapy or in patients with renal impairment)
- Complete Blood Count (CBC) (for prolonged therapy)
- Liver function tests (if signs of hepatic dysfunction appear)
- Coagulation tests (e.g., INR) (in patients concomitantly treated with anticoagulants)
Doctor Notes
- Educate parents/caregivers on the importance of completing the full course of antibiotics, even if symptoms improve, to minimize antibiotic resistance.
- Advise on proper reconstitution technique using boiled and cooled water, and the correct use of the measuring device for accurate dosing.
- Counsel patients/caregivers on potential side effects, particularly diarrhea, and when to seek medical attention (e.g., severe diarrhea, allergic reactions).
- Be aware of potential cross-reactivity in patients with a history of severe penicillin allergy.
Patient Guidelines
- Complete the full prescribed course of antibiotics, even if symptoms improve or disappear, to prevent the development of drug-resistant bacteria.
- Shake the suspension vigorously before each dose to ensure uniform distribution of the medication.
- Use the provided measuring device to ensure accurate dosing, especially for children. Do not use household spoons.
- Do not store the reconstituted suspension at room temperature; refrigerate and discard any unused portion after the specified period (e.g., 14 days).
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefixime generally does not impair the ability to drive or operate machinery. However, if side effects such as dizziness or headache occur, patients should avoid these activities until symptoms subside.
Lifestyle Advice
- Maintain good hydration throughout the course of antibiotic therapy.
- Avoid self-medication and do not share this medication with others, as it is prescribed for a specific condition.
