Overview of the medicine
C-3 200 mg Capsule is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation associated with various conditions.
Uses & Indications
Dosage
Adults
Osteoarthritis: 200 mg once daily or 100 mg twice daily. Rheumatoid Arthritis: 100-200 mg twice daily. Ankylosing Spondylitis: 200 mg once daily or 100 mg twice daily. Acute Pain/Primary Dysmenorrhea: 400 mg initially, followed by 200 mg if needed on the first day, then 200 mg twice daily.
Elderly
Generally, no dose adjustment is required in elderly patients, but caution is advised. Initiate at the lowest effective dose.
Renal_impairment
Severe renal impairment (CrCl <30 mL/min): Not recommended. Moderate impairment: Use with caution, consider reduced dose.
How to Take
C-3 capsules can be taken with or without food. To minimize potential gastrointestinal upset, it is generally recommended to take them with food.
Mechanism of Action
C-3 selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces these symptoms with a potentially lower risk of gastrointestinal side effects compared to non-selective NSAIDs.

Analgesic effects typically begin within 30-60 minutes.
Excretion
Excreted predominantly via urine (approx. 57%) and feces (approx. 27%) as metabolites; less than 3% excreted as unchanged drug.
Half life
Approximately 8-12 hours.
Absorption
Rapidly and well absorbed after oral administration, with peak plasma concentrations achieved within 2-3 hours. Food may increase absorption.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzyme CYP2C9 to inactive metabolites.

Contraindications
- Known hypersensitivity to C-3 or sulfonamides
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Severe renal or hepatic impairment
- Active peptic ulcer or gastrointestinal bleeding
- Severe heart failure
Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity.
Diuretics
May reduce natriuretic effect of loop and thiazide diuretics.
Warfarin and other anticoagulants
Increased risk of bleeding.
ACE Inhibitors / Angiotensin Receptor Blockers
May reduce antihypertensive effect and increase risk of renal impairment.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be provided. There is no specific antidote. Gastric lavage or activated charcoal may be considered within the first hour of ingestion.

Pregnancy Category C/D. Use of C-3 during the third trimester of pregnancy should be avoided due to the risk of premature closure of the fetal ductus arteriosus. Not recommended during breastfeeding due to potential excretion into breast milk.
