Overview of the medicine
Candesartan Cilexetil 8 mg tablet is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily and reducing the workload on the heart.
Uses & Indications
Dosage
Adults
For hypertension: Initial dose 8 mg once daily. May be increased to 16 mg once daily, and if necessary, further to 32 mg once daily. For heart failure: Initial dose 4 mg once daily. Dose may be doubled at intervals of at least 2 weeks to a maximum of 32 mg once daily.
Elderly
No initial dose adjustment is generally required for elderly patients.
Renal_impairment
For patients with mild to moderate renal impairment, no initial dose adjustment is necessary. For patients with severe renal impairment (creatinine clearance <30 mL/min/1.73 m²) or on hemodialysis, consider an initial dose of 4 mg once daily.
How to Take
Take Camlor 8 mg tablet orally, once daily, with or without food. Swallow the tablet whole with water. It is recommended to take it at the same time each day to maintain consistent levels in your body.
Mechanism of Action
Candesartan Cilexetil is a prodrug that is rapidly converted to the active drug candesartan during absorption. Candesartan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues, including vascular smooth muscle and the adrenal gland. By blocking the AT1 receptor, candesartan inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced sodium and water retention.
Pharmacokinetics
Onset
Onset of antihypertensive effect within 2 hours; full effect within 4 weeks.
Excretion
Approximately 33% of candesartan is excreted in urine and 67% in feces via bile, primarily as unchanged drug. Renal clearance is about 0.19 L/h/kg.
Half life
Terminal elimination half-life of candesartan is approximately 9-15 hours.
Absorption
Candesartan cilexetil is well absorbed from the gastrointestinal tract and rapidly and completely hydrolyzed to candesartan during absorption. Absolute bioavailability is approximately 15%. Peak plasma concentrations of candesartan are reached 3-4 hours after tablet intake.
Metabolism
Candesartan is minimally metabolized in the liver (O-deethylation). It is not significantly metabolized by the cytochrome P450 system.
Side Effects
Contraindications
- Pregnancy and lactation.
- Severe hepatic impairment and/or cholestasis.
- Concomitant use with aliskiren-containing products in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Aliskiren
Concomitant use in diabetic patients or patients with renal impairment (GFR <60 mL/min/1.73 m²) is contraindicated due to increased risk of hypotension, hyperkalemia, and renal function deterioration.
NSAIDs (Non-steroidal anti-inflammatory drugs)
May reduce the antihypertensive effect of candesartan and increase the risk of renal function deterioration, including possible acute renal failure, especially in elderly, volume-depleted, or renally impaired patients.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Monitor serum potassium levels.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hypotension (low blood pressure) and dizziness. Treatment is symptomatic and supportive, including close monitoring of vital signs and fluid/electrolyte balance. If ingestion is recent, gastric lavage may be considered.
Pregnancy & Lactation
Camlor is contraindicated during pregnancy due to the risk of fetal injury and death. If pregnancy is detected, discontinue Camlor immediately. It is not recommended during lactation as it is unknown whether candesartan is excreted in human milk.
