Overview of the medicine
Capazi 10 mg Tablet contains Carvedilol, a non-selective beta-adrenergic blocker with alpha1-blocking activity. It is used to treat high blood pressure, heart failure, and angina.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose 12.5 mg once daily, may be increased to 25 mg once daily. Heart failure: Initial dose 3.125 mg twice daily for two weeks, gradually increased based on tolerance.
Elderly
Similar to adult dosage, but titration should be done cautiously, especially for heart failure.
Renal_impairment
No dosage adjustment generally required for mild to moderate renal impairment; use with caution in severe cases.
How to Take
Take orally, preferably with food to slow absorption and reduce orthostatic hypotension. Do not chew or crush the tablet.
Mechanism of Action
Carvedilol selectively blocks alpha1-adrenergic receptors, causing vasodilation and reducing peripheral vascular resistance. It also non-selectively blocks beta1 and beta2-adrenergic receptors, leading to decreased heart rate, myocardial contractility, and renin release, thus lowering blood pressure and reducing cardiac workload.

Onset
Within 1 hour for blood pressure reduction.
Excretion
Primarily excreted in bile and feces, with a small amount excreted via the kidneys.
Half life
Approximately 7-10 hours.
Absorption
Rapidly and extensively absorbed after oral administration, with about 25-35% bioavailability due to significant first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP2C9, to various metabolites, some of which are active.
Side Effects
Contraindications
- •Bronchial asthma or related bronchospastic conditions
- •Second- or third-degree AV block (unless permanent pacemaker is in place)
- •Sick sinus syndrome (unless permanent pacemaker is in place)
- •Severe bradycardia (heart rate <50 bpm)
- •Cardiogenic shock or decompensated heart failure requiring IV inotropic therapy
- •Severe hepatic impairment
Drug Interactions
Digoxin
May increase digoxin concentrations and lead to increased AV block.
Clonidine
Severe bradycardia and hypotension if abruptly withdrawn.
Cimetidine
May increase carvedilol plasma concentrations.
Rifampicin
May decrease carvedilol plasma concentrations.
Insulin/Oral Hypoglycemics
May enhance the blood sugar lowering effect and mask symptoms of hypoglycemia.
Storage
Store in a cool, dry place below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe hypotension, bradycardia, cardiac insufficiency, cardiogenic shock, and bronchospasm. Treatment involves symptomatic and supportive measures.
Pregnancy & Lactation
Pregnancy Category C (risk cannot be ruled out). Should only be used if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk.
