Description
Overview of the medicine
Capazi 5 mg Tablet is used to treat high blood pressure (hypertension) and heart failure. It belongs to a class of medicines called Angiotensin II Receptor Blockers (ARBs). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Hypertension: Initially 8 mg once daily, adjusted according to response; usual maintenance dose 8-32 mg once daily. For Capazi 5 mg, it may be used as a starting dose in some populations or for mild cases, however, typical starting doses for Candesartan are 8 mg. Heart failure: Initially 4 mg once daily, increasing at intervals of at least 2 weeks to 8 mg, 16 mg, then 32 mg once daily as tolerated.
Elderly
No initial dose adjustment is generally required in elderly patients.
Renal_impairment
In mild to moderate renal impairment, no initial dose adjustment is needed. For severe renal impairment or hemodialysis patients, a lower starting dose (e.g., 4 mg) may be considered, and careful monitoring is advised.
How to Take
Take Capazi 5 mg Tablet orally, once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Capazi (Candesartan Cilexetil) is an Angiotensin II Receptor Blocker (ARB). It selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to a reduction in blood pressure.
Pharmacokinetics
Onset
Blood pressure lowering effect starts within 2 hours; maximum reduction is achieved within 4-6 weeks.
Excretion
Excreted unchanged in urine (33%) and bile (67%).
Half life
Approximately 9 hours for candesartan.
Absorption
Candesartan cilexetil is an inactive prodrug that is rapidly and completely hydrolyzed to candesartan during absorption from the gastrointestinal tract. Absolute bioavailability of candesartan is approximately 15%. Peak plasma concentrations (Cmax) are reached within 3-4 hours.
Metabolism
Candesartan is not significantly metabolized by the cytochrome P450 system. It is primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to candesartan cilexetil or any component of the formulation.
- Pregnancy (second and third trimesters).
- Severe hepatic impairment and/or cholestasis.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Aliskiren
Contraindicated in patients with diabetes or renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of ARBs and may worsen renal function, especially in elderly or volume-depleted patients. Monitor renal function.
Potassium-sparing diuretics, potassium supplements, or other agents that increase potassium levels
May lead to hyperkalemia. Monitor serum potassium levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension, dizziness, and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage may be considered. Administer activated charcoal. Candesartan is not removed by hemodialysis.
Pregnancy & Lactation
Capazi 5 mg Tablet is contraindicated during the second and third trimesters of pregnancy due to the risk of fetal harm and death. Discontinue if pregnancy is detected. It is not recommended during lactation as it is unknown whether candesartan is excreted in human milk.
