Description
Overview of the medicine
Cardon is a beta-blocker and alpha-blocker used to treat high blood pressure, heart failure, and left ventricular dysfunction after a heart attack. It works by relaxing blood vessels and slowing heart rate to improve blood flow and lower blood pressure.
Uses & Indications
Dosage
Adults
Hypertension: Initially 6.25 mg twice daily, increased to 12.5 mg then 25 mg twice daily as needed. Heart failure: Initially 3.125 mg twice daily, gradually titrated up to a maximum of 25 mg twice daily for patients under 85 kg or 50 mg twice daily for patients over 85 kg.
Elderly
No specific dose adjustment needed, but titration should be slow and cautious.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment, but caution is advised.
How to Take
Take orally, preferably with food to enhance absorption and reduce the risk of orthostatic hypotension.
Mechanism of Action
Carvedilol is a non-selective beta-adrenergic receptor blocker and an alpha1-adrenergic receptor blocker. It reduces heart rate and cardiac output, lowers peripheral vascular resistance, and inhibits renin release from the kidney.
Pharmacokinetics
Onset
Approximately 1 hour for blood pressure reduction.
Excretion
Primarily excreted in feces via bile, with a small amount excreted in urine.
Half life
Approximately 7-10 hours.
Absorption
Rapidly absorbed after oral administration, bioavailability is about 25-35% due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP2C9, to active and inactive metabolites.
Side Effects
Contraindications
- •Bronchial asthma or related bronchospastic conditions
- •Second or third-degree AV block (unless a permanent pacemaker is in place)
- •Sick sinus syndrome (unless a permanent pacemaker is in place)
- •Cardiogenic shock
- •Severe bradycardia
- •Decompensated heart failure requiring intravenous inotropic therapy
- •Severe hepatic impairment
Drug Interactions
Digoxin
Increased digoxin levels and enhanced bradycardia.
Clonidine
Potential for rebound hypertension if clonidine is withdrawn abruptly while on carvedilol.
Cimetidine
Increased carvedilol bioavailability.
Rifampicin
Decreased carvedilol plasma concentrations.
Insulin/Oral Hypoglycemics
May enhance the hypoglycemic effect and mask symptoms of hypoglycemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension, bradycardia, cardiac insufficiency, cardiogenic shock, and bronchospasm. Treatment is supportive, including atropine for bradycardia, vasopressors for hypotension, and glucagon for severe cases.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy unless the potential benefit justifies the potential risk to the fetus. Excreted in human milk, so caution is advised during breastfeeding.
