Overview of the medicine
Cavazide 150 mg Tablet is a combination medicine used to treat high blood pressure (hypertension). It contains an Angiotensin II Receptor Blocker (Irbesartan) and a thiazide diuretic (Hydrochlorothiazide) which work together to lower blood pressure effectively.
Uses & Indications
Dosage
Adults
Usually one tablet (Irbesartan 150 mg / Hydrochlorothiazide 12.5 mg) once daily. May be increased if blood pressure is not controlled.
Elderly
Initial dose adjustment not usually required, but monitor renal function.
Renal_impairment
Not recommended in severe renal impairment (CrCl < 30 mL/min). Dose adjustment may be necessary in moderate impairment.
How to Take
Oral, usually once daily with or without food.
Mechanism of Action
Irbesartan blocks AT1 receptors, reducing vasoconstriction and aldosterone secretion. Hydrochlorothiazide increases excretion of sodium and water by inhibiting sodium reabsorption in the distal convoluted tubule.

Irbesartan: ~1.5 hours. Hydrochlorothiazide: ~2 hours. Peak effect 6-12 hours.

Irbesartan: Biliary and renal. Hydrochlorothiazide: Primarily renal.

Side Effects
Contraindications
- Hypersensitivity to irbesartan, hydrochlorothiazide, or sulfonamide-derived drugs.
- Anuria.
- Pregnancy (second and third trimesters).
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
NSAIDs
May reduce antihypertensive effect and increase risk of renal impairment.
Lithium
Increased serum lithium levels and toxicity.
Aliskiren
Contraindicated in patients with diabetes or renal impairment.
Corticosteroids
May enhance potassium depletion (with hydrochlorothiazide component).
Potassium-sparing diuretics/potassium supplements
May increase serum potassium (with irbesartan component).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypotension, dizziness, tachycardia, and electrolyte disturbances (e.g., hypokalemia, hyponatremia). Treatment is supportive and symptomatic; hemodialysis is not effective for irbesartan.
Pregnancy & Lactation
Contraindicated in the 2nd and 3rd trimesters of pregnancy due to fetal toxicity. Not recommended during lactation as both components are excreted in breast milk.
