Description
Overview of the medicine
Ceclav 250 mg Tablet is a broad-spectrum antibiotic combination used to treat various bacterial infections. It contains Cefixime, a third-generation cephalosporin antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor, which enhances Cefixime's antibacterial activity against resistant bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is Cefixime 200 mg + Clavulanic Acid 50 mg twice daily. The duration of treatment depends on the infection type and severity, typically 7-14 days.
Elderly
No dosage adjustment required in elderly patients with normal renal function. Monitor renal function.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (CrCl < 30 mL/min). Consult physician for specific recommendations.
How to Take
Take the tablet orally with or without food. It is generally recommended to take it with food to reduce gastrointestinal upset. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Clavulanic Acid inactivates a wide range of beta-lactamase enzymes produced by bacteria, which would otherwise hydrolyze cefixime, thereby protecting cefixime from degradation and extending its spectrum of activity.
Pharmacokinetics
Onset
Typically, within 1-2 hours for therapeutic levels.
Excretion
Cefixime is primarily excreted via urine (approximately 50% as unchanged drug) and bile. Clavulanic Acid is primarily excreted in urine.
Half life
Cefixime: 3-4 hours; Clavulanic Acid: approximately 1 hour.
Absorption
Cefixime is approximately 40-50% absorbed from the GI tract. Clavulanic Acid is well-absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Cefixime is not extensively metabolized; Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to cefixime, clavulanic acid, any other cephalosporin antibiotics, or penicillin antibiotics.
- •History of severe gastrointestinal disease, especially colitis.
Drug Interactions
Probenecid
May increase cefixime plasma levels by decreasing renal tubular secretion.
Carbamazepine
Cefixime may increase carbamazepine levels, monitor levels and adjust dosage if needed.
Anticoagulants (e.g., Warfarin)
Increased prothrombin time (PT) / INR, increased risk of bleeding. Monitor coagulation parameters.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and central nervous system excitability. Treatment is supportive and symptomatic; hemodialysis or peritoneal dialysis is not effective in removing significant amounts of cefixime.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed during pregnancy. Small amounts of cefixime are excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
