Description
Overview of the medicine
Cefadroxil is a first-generation cephalosporin antibiotic used to treat various bacterial infections susceptible to it, such as urinary tract infections, skin and soft tissue infections, and respiratory tract infections.
Uses & Indications
Dosage
Adults
Not typically used for adults in this specific strength and formulation; usually 1-2 g/day in 1-2 divided doses in other forms. For suspension, consult physician for specific dosage based on weight and infection type.
Elderly
Dosage adjustment may be required in elderly patients with impaired renal function. Monitor renal function.
Renal_impairment
Dosage should be reduced based on creatinine clearance (CrCl). For CrCl 50 ml/min: 500 mg every 12 hours; for CrCl 25-49 ml/min: 500 mg every 24 hours; for CrCl 10-24 ml/min: 500 mg every 36 hours. Patients on dialysis: 500 mg every 12-24 hours.
How to Take
For oral administration. Shake the bottle well before each use. It can be taken with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefadroxil inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 1-2 hours after oral administration.
Excretion
Mainly excreted unchanged in the urine via glomerular filtration and tubular secretion.
Half life
Approximately 1.5-2 hours in individuals with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, even in the presence of food.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefadroxil, other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
May increase cefadroxil plasma concentrations by decreasing renal tubular secretion.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity, especially in patients with pre-existing renal impairment.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect; monitor INR carefully.
Storage
Store dry powder below 30°C, away from moisture and direct light. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management is primarily supportive and symptomatic. Gastric lavage or activated charcoal may be considered in symptomatic patients. Hemodialysis may be useful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not demonstrated a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use this medicine during pregnancy only if clearly needed. Cefadroxil is excreted in small amounts in breast milk; use with caution in nursing mothers.

Lab Monitoring
- Renal function tests (blood urea nitrogen, serum creatinine) should be monitored, especially in patients with pre-existing renal impairment.
- Periodic complete blood counts (CBC) may be advisable during prolonged therapy.
- Liver function tests if signs of hepatic dysfunction develop.
Doctor Notes
- Counsel patients on the importance of completing the full course of antibiotic therapy to prevent resistance and recurrence.
- Advise parents/caregivers on proper measurement and administration of the suspension, including shaking well before use and appropriate storage.
- Be alert for signs of hypersensitivity reactions, especially in patients with a history of penicillin allergy.
Patient Guidelines
- Complete the entire prescribed course of medication, even if symptoms improve or disappear, to prevent the development of antibiotic resistance.
- Shake the oral suspension well before each dose.
- Store the reconstituted suspension in the refrigerator and discard any unused portion after the specified period (e.g., 14 days).
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefadroxil is generally not expected to impair the ability to drive or operate machinery. However, if side effects such as dizziness occur, patients should avoid these activities.
Lifestyle Advice
- Maintain good personal hygiene to help prevent infections.
- Stay well-hydrated by drinking plenty of fluids, especially if experiencing diarrhea.
