Overview of the medicine
Cefamax 200 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic, used to treat various bacterial infections like respiratory tract infections, urinary tract infections, and gonorrhea.
Uses & Indications
Dosage
Adults
200-400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No dose adjustment required unless there is severe renal impairment.
Renal_impairment
Dose adjustment necessary for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of the standard dose; for CrCl < 20 mL/min, 50% of the standard dose.
How to Take
Cefamax capsule can be taken with or without food. To minimize gastrointestinal upset, it is generally recommended to take it with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cytoplasmic membrane, thereby interfering with the transpeptidation reaction of peptidoglycan synthesis, which is crucial for bacterial cell wall formation. This leads to the lysis and death of susceptible bacteria.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (approximately 50% of the absorbed dose) and also via biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slightly delays absorption but does not significantly alter the extent.
Metabolism
Not extensively metabolized in the body; about 10% of dose is metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or other cephalosporins.
- •History of severe hypersensitivity reactions to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase prothrombin time (PT) and INR, increasing the risk of bleeding. Close monitoring of coagulation parameters is advised.
Probenecid
Increases cefixime plasma concentrations and prolongs its half-life by decreasing renal excretion.
Carbamazepine
Increased plasma levels of carbamazepine have been reported when co-administered with cefixime.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
