Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used for adults in this specific strength and formulation. Adult dosage usually 400 mg once daily.
Elderly
No specific dosage adjustment needed unless severe renal impairment (Creatinine Clearance < 60 mL/min).
Children
6 months to 12 years: 8 mg/kg/day once daily or in two divided doses. For Cefamax 90 mg Suspension, dose should be calculated based on body weight.
Renal_impairment
Dosage adjustment required for patients with Creatinine Clearance < 60 mL/min. For CrCl 21-60 mL/min or on peritoneal dialysis, dose reduction to 75% of standard. For CrCl < 20 mL/min or on hemodialysis, dose reduction to 50% of standard.
How to Take
Administer orally with or without food. Shake the bottle well before each use to ensure uniform dosage. Use a calibrated measuring spoon or cup provided with the package.
Mechanism of Action
Cefixime works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death.
Pharmacokinetics
Onset
Within 1 hour of oral administration.
Excretion
Mainly renal excretion (50-60% unchanged in urine); some biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Limited metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •Known hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
May increase cefixime levels (though clinical significance is generally minor).
Carbamazepine
Rarely, increased carbamazepine levels have been reported with concurrent cefixime use.
Warfarin and other oral anticoagulants
Concurrent use may increase prothrombin time (INR), requiring close monitoring and possible anticoagulant dosage adjustment.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep out of reach of children. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after 14 days. Do not freeze.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate human studies. Use only if clearly needed. Cefixime is excreted in breast milk in small amounts; caution should be exercised when administered to a nursing mother.
