Overview of the medicine
Cefixime is a broad-spectrum, third-generation cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours. Duration depends on infection type, usually 7-14 days.
Elderly
No specific dose adjustment is required unless there is severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min: 300 mg daily. For creatinine clearance <20 mL/min: 200 mg daily.
How to Take
Ceficap-400-mg-capsule can be taken orally with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Onset of antibacterial action typically within 1-2 hours.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine; a small portion is excreted via bile.
Half life
Plasma half-life is usually 3-4 hours, potentially longer in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Peak plasma concentrations are reached 2-6 hours after administration.
Metabolism
Not extensively metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- Known hypersensitivity to penicillin antibiotics (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase prothrombin time (PT) and INR, increasing risk of bleeding. Close monitoring is recommended.
Nifedipine
May increase cefixime absorption.
Probenecid
May increase cefixime plasma concentrations and prolong its half-life by decreasing renal excretion.
Carbamazepine
Concomitant use may increase carbamazepine plasma levels. Monitor carbamazepine levels.
Storage
Store at room temperature (below 30°C) in a dry place, away from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Cefixime is excreted in human milk in small amounts; use with caution in breastfeeding mothers.
