Overview of the medicine
Cefida (Cephradine) is a first-generation cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, skin, and soft tissue.
Uses & Indications
Dosage
Adults
250-500 mg every 6-12 hours, depending on the severity of infection. Maximum 4 g/day.
Elderly
Dosage adjustment may be required in elderly patients with impaired renal function. Monitor renal function.
Renal_impairment
Reduced dosage based on creatinine clearance (e.g., for CrCl 10-20 mL/min, 250 mg every 12-24 hours).
How to Take
Cefida capsules should be taken orally, with or without food. It is recommended to take them with a full glass of water.
Mechanism of Action
Cephradine works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death.
Pharmacokinetics
Onset
Clinical effects typically begin within 1 hour after administration.
Excretion
Primarily excreted unchanged in the urine, mainly by glomerular filtration and tubular secretion. Approximately 80-90% of a dose is excreted within 6 hours.
Half life
Approximately 1-2 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Not extensively metabolized in the body.
Side Effects
Contraindications
- •Known hypersensitivity to cephradine or any other cephalosporin antibiotics.
- •History of severe allergic reaction to penicillin (due to potential cross-reactivity).
Drug Interactions
Probenecid
Concomitant use with probenecid may increase and prolong plasma concentrations of cephradine by inhibiting its renal tubular secretion.
Aminoglycosides
Increased risk of nephrotoxicity when administered concurrently with aminoglycosides, especially in patients with pre-existing renal impairment.
Oral anticoagulants
Cephalosporins may enhance the anticoagulant effect of warfarin and other oral anticoagulants.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management is primarily supportive; gastric lavage or activated charcoal may be considered in recent ingestions. Hemodialysis may be useful in removing cephradine from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Cephradine is excreted in breast milk in low concentrations; use with caution in nursing mothers.
