Cefoxil 125 mg Suspension contains Cefuroxime Axetil, which is an oral prodrug of the bactericidal cephalosporin antibiotic cefuroxime. It is used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used for adults in 125 mg suspension form; adult dosage usually 250-500 mg twice daily in tablet form.
Elderly
No specific dosage adjustment required for elderly patients with normal renal function.
Children
Most infections: 125 mg (5 ml) twice daily. For otitis media or more severe infections: 250 mg (10 ml) twice daily. Dosage may also be based on weight (e.g., 10-15 mg/kg twice daily).
Renal_impairment
Dosage adjustment is necessary in patients with significant renal impairment. Refer to prescribing information for specific creatinine clearance based dosing.
How to Take
Administer orally, preferably with food to enhance absorption and reduce gastrointestinal discomfort. Shake the bottle well before each use to ensure uniform dosage.
Mechanism of Action
Cefuroxime Axetil is hydrolyzed to cefuroxime in the body, which then inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Rapid, typically within 1 hour.
Excretion
Primarily excreted unchanged via the kidneys by glomerular filtration and tubular secretion. Approximately 50% of the dose is excreted within 12 hours.
Half life
Approximately 1.2 to 1.5 hours in adults with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract, enhanced when taken with food. Peak plasma concentrations occur 2-3 hours after oral administration.
Metabolism
Hydrolyzed to cefuroxime in the intestinal mucosa and blood. Cefuroxime itself is not further metabolized.
Side Effects
Contraindications
- •Known hypersensitivity to cefuroxime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agents (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral anticoagulants
Concurrent use may increase INR (International Normalised Ratio) and requires close monitoring.
Oral contraceptives
May reduce the efficacy of oral contraceptives; additional barrier contraception may be advised.
Antacids/H2 antagonists/PPIs
Drugs that reduce gastric acidity may decrease the bioavailability of cefuroxime axetil and should be used with caution.
Storage
Unreconstituted powder: Store below 30°C in a dry place, away from light. Reconstituted suspension: Store in a refrigerator (2-8°C) and discard any unused portion after 10 days.
Overdose
Overdose of cephalosporins can lead to cerebral irritation, which may result in convulsions. Management includes gastric lavage or emesis, supportive therapy, and monitoring of vital signs. Hemodialysis and peritoneal dialysis can aid in removal.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Cefuroxime is excreted in small amounts in human breast milk; use with caution during lactation. Consult a doctor.
