Description
Overview of the medicine
Cefalexin is a first-generation cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, skin and soft tissue, and bone.
Uses & Indications
Dosage
Adults
250-500 mg every 6 hours or 500 mg every 12 hours, depending on the severity of infection. Max 4 g/day.
Elderly
Similar to adult dosage, but dose reduction may be necessary in patients with significant renal impairment.
Renal_impairment
Dosage adjustment required based on creatinine clearance (e.g., lower doses or extended intervals).
How to Take
May be taken with or without food. To minimize gastrointestinal upset, it can be taken with food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefalexin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 1 hour after oral administration.
Excretion
Over 90% of the drug is excreted unchanged in the urine within 8 hours by glomerular filtration and tubular secretion.
Half life
Approximately 0.5 to 1.2 hours in individuals with normal renal function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Food may delay absorption but does not significantly reduce the total amount absorbed.
Metabolism
Cefalexin is minimally metabolized in the liver; it is primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to Cefalexin or any other cephalosporin antibiotics
- •History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring monitoring of INR.
Metformin
Cefalexin may increase plasma concentrations of metformin, requiring dose adjustment.
Probenecid
Increases plasma concentrations of cefalexin by decreasing renal tubular secretion.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. Treatment is primarily supportive. Gastric lavage and activated charcoal may be considered. Hemodialysis or peritoneal dialysis may be helpful.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
