Overview of the medicine
Ceframed 250 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
250 mg twice daily, or 500 mg once daily (two 250 mg capsules), depending on the infection. Duration typically 7-14 days.
Elderly
No specific dose adjustment generally needed unless severe renal impairment is present. Monitor renal function.
Renal_impairment
Dose adjustment required for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of standard dose. For CrCl < 20 mL/min, 50% of standard dose.
How to Take
Take orally with or without food. Swallow the capsule whole with a glass of water. It can be taken at the same time each day for best effect.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations usually within 2-6 hours post-dose.
Excretion
Primarily excreted via urine (50-55% of absorbed dose) and bile/feces.
Half life
3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay but not significantly alter absorption. Peak plasma concentrations reached within 2-6 hours.
Metabolism
Not significantly metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
May increase cefixime Cmax and AUC, though clinical significance is usually minimal.
Carbamazepine
Cefixime may increase carbamazepine levels when co-administered. Monitor carbamazepine levels.
Oral Contraceptives
Potential for reduced efficacy of oral contraceptives (though controversial, advisable to use alternative contraception).
Warfarin and other anticoagulants
Concurrent use may increase prothrombin time (INR) and bleeding risk. Close monitoring is recommended.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Hemodialysis or peritoneal dialysis is not effective in removing cefixime. Treatment should be symptomatic and supportive.
