Overview of the medicine
Ceftazidime is a third-generation cephalosporin antibiotic used to treat a variety of serious bacterial infections, especially those caused by Gram-negative bacteria like Pseudomonas aeruginosa.
Uses & Indications
Dosage
Adults
For less severe infections, typically 250 mg to 1 g IV/IM every 8-12 hours, depending on infection severity and patient factors. Higher doses are usually 1-2g every 8-12 hours. The 250 mg strength is often used in children or for specific dose adjustments.
Elderly
Dosage adjustments may be necessary based on renal function, as elderly patients may have reduced renal clearance.
Renal_impairment
Dosage reduction is required based on creatinine clearance. For CrCl 31-50 mL/min: 1 g every 12 hours. For CrCl 16-30 mL/min: 1 g every 24 hours. For CrCl 6-15 mL/min: 500 mg every 24 hours. For CrCl <5 mL/min: 500 mg every 48 hours.
How to Take
Administered intravenously (IV) as a slow injection over 3-5 minutes or by infusion, or intramuscularly (IM) into a large muscle mass after reconstitution with appropriate diluent (e.g., sterile water for injection).
Mechanism of Action
Ceftazidime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, within minutes for IV administration; 30 minutes for IM administration.
Excretion
Predominantly renal (glomerular filtration and tubular secretion) as unchanged drug. Approximately 80-90% is excreted in the urine within 24 hours.
Half life
Approximately 1.8 to 2 hours in adults with normal renal function.
Absorption
Rapid and complete absorption after intramuscular (IM) administration; high bioavailability intravenously (IV). Peak plasma concentrations occur within 1 hour after IM injection.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to ceftazidime, other cephalosporins, or any component of the formulation.
- •History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Loop diuretics
High doses may increase cephalosporin levels; caution is advised.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity. Monitor renal function closely.
Chloramphenicol
May antagonize the bactericidal effect of ceftazidime. Avoid concomitant use.
Oral anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, increasing the risk of bleeding. Monitor INR/PT.
Storage
Store the dry powder below 30°C in a dry place, protected from light. Reconstituted solution should be used immediately or stored in a refrigerator (2-8°C) for up to 24 hours.
Overdose
Symptoms of overdose may include convulsions, encephalopathy, neuromuscular excitability, and asterixis, particularly in patients with renal impairment. Management is supportive; hemodialysis or peritoneal dialysis may be useful to remove ceftazidime from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use in pregnancy only if clearly needed. Use with caution in nursing mothers as low levels are excreted in breast milk.
