Description
Overview of the medicine
Cefteria (Ceftriaxone) is a broad-spectrum, third-generation cephalosporin antibiotic used to treat various bacterial infections. It works by inhibiting bacterial cell wall synthesis.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2 gm administered once daily (or in equally divided doses twice daily) either intravenously or intramuscularly. In severe infections, the total daily dose may be increased to 4 gm.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is required in patients with renal impairment, provided hepatic function is not impaired. In cases of severe renal and hepatic dysfunction, the dose should not exceed 2 gm daily.
How to Take
Cefteria is administered by deep intramuscular injection or by slow intravenous infusion over 30 minutes. Reconstitute the powder with sterile water for injection or lidocaine for IM.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid (within minutes for IV administration).
Excretion
Approximately 50-60% excreted unchanged in urine and 40-50% excreted unchanged in bile.
Half life
Approximately 6 to 9 hours.
Absorption
Administered intravenously (IV) or intramuscularly (IM), so absorption phase is bypassed for IV. IM absorption is rapid and complete.
Metabolism
Not extensively metabolized; primarily converted into inactive metabolites in the gastrointestinal tract by bacterial flora.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, any other cephalosporin, or to any of the excipients.
- •Patients with a history of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams and carbapenems).
- •Neonates with hyperbilirubinemia, especially premature neonates.
Drug Interactions
Calcium-containing solutions
Contraindicated in neonates; risk of precipitation.
Anticoagulants (e.g., Warfarin)
May increase the risk of bleeding.
Loop diuretics (e.g., Furosemide)
Potential for increased nephrotoxicity when co-administered with other nephrotoxic drugs, though Ceftriaxone itself is generally not nephrotoxic.
Storage
Store in a cool and dry place, below 25°C, protected from light. Keep out of reach of children.
Overdose
In the case of overdose, symptoms are generally an exaggeration of the reported adverse reactions. There is no specific antidote. Treatment should be symptomatic and supportive. Hemodialysis and peritoneal dialysis will not remove Ceftriaxone effectively.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown evidence of impaired fertility or harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Ceftriaxone is excreted in small quantities in human breast milk; caution should be exercised when administered to a nursing woman.
