Cefuroxime Axetil is a broad-spectrum, second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, skin and soft tissues.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily. More severe infections or bronchitis/pneumonia: 500 mg twice daily. Uncomplicated gonorrhea: 1000 mg as a single dose.
Elderly
No specific dose adjustment needed based solely on age, but consider renal function.
Renal_impairment
For creatinine clearance < 30 mL/min, dosage adjustment is required. Consult prescribing information for specific recommendations.
How to Take
Take Cefuact 250 mg Tablet orally with food to optimize absorption and reduce potential gastrointestinal upset. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Cefuroxime Axetil acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.

Absorption
Rapidly absorbed from the gastrointestinal tract and hydrolyzed to active cefuroxime. Absorption is enhanced when taken with food.

Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or to any of the excipients.
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams, carbapenems).
Drug Interactions
Warfarin
Concomitant use with oral anticoagulants may cause an increase in INR. Careful monitoring is advised.
Probenecid
Concomitant administration with probenecid increases the area under the plasma concentration-time curve of cefuroxime and prolongs its duration of action.
Oral Contraceptives
Cefuroxime may affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives. Advise additional barrier contraception.
Antacids/H2 Blockers
Drugs that reduce gastric acidity can lead to a lower bioavailability of Cefuroxime Axetil. Administer Cefuroxime Axetil at least 2 hours before or after antacids or H2 blockers.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include cerebral irritation leading to convulsions. Management is primarily supportive; hemodialysis or peritoneal dialysis may be considered to remove cefuroxime from the body, especially in cases of severe renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefuroxime is excreted in small amounts in breast milk; caution is advised when administering to a nursing mother. Monitor breastfed infants for potential adverse effects like diarrhea or thrush.
