Overview of the medicine
Cefurav-Plus 250 mg tablet is a broad-spectrum antibiotic containing Cefuroxime Axetil and Clavulanic Acid. It is used to treat various bacterial infections susceptible to this combination, where the beta-lactamase inhibitory action of clavulanic acid protects cefuroxime from degradation.
Uses & Indications
Dosage
Adults
One 250 mg tablet twice daily for 7-10 days, or as directed by the physician. For more severe infections, dosage may be increased.
Elderly
No specific dose adjustment is usually required unless there is significant renal impairment.
Renal_impairment
Dose adjustment is necessary for severe renal impairment (CrCl < 30 mL/min). Consult a physician for specific dosage.
How to Take
Taken orally, preferably with food to enhance absorption and reduce gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Cefuroxime is a second-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Clavulanic acid is a beta-lactamase inhibitor that inactivates a wide range of beta-lactamase enzymes produced by bacteria, thereby protecting cefuroxime from enzymatic hydrolysis and extending its antibacterial spectrum.
Pharmacokinetics
Onset
Rapid onset, typically within 1-2 hours for therapeutic levels.
Excretion
Both cefuroxime and clavulanic acid are primarily excreted by the kidneys, mostly unchanged for cefuroxime, and as metabolites for clavulanic acid.
Half life
Cefuroxime: Approximately 1-1.5 hours. Clavulanic acid: Approximately 1 hour.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is increased with food. Peak plasma concentrations occur 2-3 hours after oral administration. Clavulanic acid is also well absorbed orally, with peak concentrations occurring in about 1 hour.
Metabolism
Cefuroxime axetil is hydrolyzed by esterases in the intestinal mucosa and blood to active cefuroxime. It is not significantly metabolized further in the liver. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to cefuroxime, clavulanic acid, any other cephalosporins, penicillins, or any component of the formulation.
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other beta-lactam antibacterial agent.
Drug Interactions
Probenecid
Concomitant use with probenecid increases cefuroxime plasma concentrations and prolongs its half-life.
Oral Anticoagulants
May potentiate the anticoagulant effect of warfarin and other oral anticoagulants.
Oral Contraceptives
May decrease the efficacy of oral contraceptives (advise additional contraceptive methods).
Antacids/H2 Blockers/PPIs
May decrease the bioavailability of cefuroxime axetil, especially if taken simultaneously.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal disturbances (nausea, vomiting, diarrhea) and possibly neurological effects like convulsions in renally impaired patients. Management is symptomatic and supportive. Hemodialysis can remove cefuroxime from the circulation.
Pregnancy & Lactation
Pregnancy category B. While generally considered safe, use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Both cefuroxime and clavulanic acid are excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
