Overview of the medicine
Cefurav-Plus 500 mg Tablet is a combination antibiotic containing Cefuroxime Axetil and Clavulanic Acid. Cefuroxime is a second-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis. Clavulanic Acid is a beta-lactamase inhibitor that protects cefuroxime from degradation by beta-lactamase enzymes produced by some bacteria, thereby extending its antibacterial spectrum.
Uses & Indications
Dosage
Adults
One Cefurav-Plus 500 mg tablet (Cefuroxime Axetil 500 mg + Clavulanic Acid 125 mg) twice daily for 7-10 days, or as directed by physician.
Elderly
Dosage adjustment may be required based on renal function. Use with caution.
Renal_impairment
Dose adjustment necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). Consult a physician for specific recommendations.
How to Take
Take the tablet orally with food for optimal absorption and to minimize gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Cefuroxime acts by inhibiting bacterial cell wall synthesis, leading to cell lysis and death. Clavulanic Acid is an irreversible inhibitor of beta-lactamase enzymes, preventing the breakdown of cefuroxime and restoring its activity against beta-lactamase producing strains.
Pharmacokinetics
Onset
Peak plasma concentrations are typically achieved 2-3 hours after oral administration.
Excretion
Both cefuroxime and clavulanic acid are primarily eliminated by renal excretion (glomerular filtration and tubular secretion).
Half life
Cefuroxime: approximately 1.2-1.5 hours. Clavulanic Acid: approximately 1 hour.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Oral bioavailability is approximately 37% to 52%. Clavulanic acid is also well absorbed orally.
Metabolism
Cefuroxime is not metabolized. Clavulanic acid undergoes extensive metabolism.
Side Effects
Contraindications
- •Known hypersensitivity to cefuroxime, clavulanic acid, any other cephalosporins, penicillins, or other beta-lactam antibiotics.
- •History of severe allergic reaction to any beta-lactam antibiotic.
Drug Interactions
Probenecid
Reduces the renal clearance of cefuroxime, leading to increased and prolonged plasma concentrations of cefuroxime.
Oral Anticoagulants
May potentiate the anticoagulant effect, increasing the risk of bleeding. Close monitoring of INR is recommended.
Antacids/H2 Receptor Blockers
May reduce the bioavailability of cefuroxime axetil, so administration should be separated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of Cefurav-Plus may cause gastrointestinal disturbances such as nausea, vomiting, and diarrhea. Management should be symptomatic and supportive. Hemodialysis may be considered to aid in the removal of the drug from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use during pregnancy only if clearly needed after consulting a doctor. Cefuroxime is excreted in breast milk in small quantities; caution should be exercised when administered to a nursing mother.
