Description
Overview of the medicine
Cefurim 250 mg tablet contains Cefuroxime axetil, a broad-spectrum second-generation cephalosporin antibiotic. It is effective against a wide range of gram-positive and gram-negative bacteria and is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily. Severe infections or bronchitis: 500 mg twice daily. Uncomplicated UTIs: 125-250 mg twice daily. Uncomplicated gonorrhea: 1000 mg as a single dose.
Elderly
Similar to adult dosage; dose adjustment may be required in patients with significantly impaired renal function.
Renal_impairment
Creatinine clearance 10-29 mL/min: 250 mg once daily. Creatinine clearance <10 mL/min or on hemodialysis: 250 mg every 48 hours or after dialysis.
How to Take
Take orally with food to optimize absorption and reduce gastrointestinal upset.
Mechanism of Action
Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bactericidal activity and cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations achieved in approximately 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Approximately 1.2 to 1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases to active cefuroxime. Food enhances absorption.
Metabolism
Cefuroxime itself is not metabolized; the axetil ester is hydrolyzed to cefuroxime.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or to any component of the formulation.
- History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., penicillin, carbapenems).
Drug Interactions
Warfarin
May enhance anticoagulant effect; monitor INR.
Probenecid
Increases and prolongs cefuroxime serum concentrations.
Oral Contraceptives
May reduce efficacy of estrogen-containing oral contraceptives; advise use of alternative birth control.
Antacids/H2 blockers/PPIs
May reduce bioavailability; administer cefuroxime at least 2 hours before or after these agents.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose can lead to neurological sequelae including seizures. Management involves supportive care and symptomatic treatment. Hemodialysis can remove cefuroxime from the blood.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution in nursing mothers.
