Description
Overview of the medicine
Triamcinolone Acetonide is a synthetic corticosteroid with potent anti-inflammatory, immunosuppressive, and anti-allergic properties, used to treat various conditions requiring systemic corticosteroid therapy.
Uses & Indications
Dosage
Adults
The usual starting dose for adults is 40-60 mg as a single intramuscular injection, repeated every 4-6 weeks as needed. Dosage varies based on the severity of the condition and patient response.
Elderly
Similar to adults, but use with caution due to increased susceptibility to side effects. The lowest effective dose should be used.
Renal_impairment
No specific dose adjustment is usually required for renal impairment, but caution is advised in severe cases due to potential fluid retention.
How to Take
For intramuscular injection only. Administer deeply into the gluteal muscle to avoid local tissue atrophy. Not for intravenous, intradermal, intraocular, or intralesional use.
Mechanism of Action
Triamcinolone Acetonide binds to specific intracellular glucocorticoid receptors, forming a complex that translocates into the nucleus. This complex modulates gene expression, leading to the synthesis of anti-inflammatory proteins (e.g., lipocortins) and inhibition of pro-inflammatory mediators (e.g., prostaglandins, leukotrienes). It also suppresses immune cell function and proliferation.
Pharmacokinetics
Onset
The onset of anti-inflammatory effects usually occurs within 24-48 hours, with clinical benefit lasting for several weeks.
Excretion
Excreted mainly in the urine as metabolites, with a small portion excreted in the feces.
Half life
Plasma half-life is approximately 2-5 hours, but biological half-life ranges from 18-36 hours due to tissue binding and slow elimination.
Absorption
Rapidly and completely absorbed after intramuscular injection. Peak plasma concentrations are reached within 8-24 hours.
Metabolism
Primarily metabolized in the liver to inactive metabolites by CYP3A4 enzymes.
Side Effects
Contraindications
- •Systemic fungal infections
- •Hypersensitivity to triamcinolone or any component of the formulation
- •Administration of live or live attenuated vaccines in patients receiving immunosuppressive doses of corticosteroids
Drug Interactions
Live vaccines
Should not be given to patients receiving immunosuppressive doses of corticosteroids due to risk of disseminated infection.
Anticoagulants (e.g., Warfarin)
May enhance or decrease anticoagulant effects; monitor INR.
CYP3A4 Inducers (e.g., Phenytoin, Rifampin)
May decrease corticosteroid levels, reducing efficacy.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of gastrointestinal ulceration and bleeding.
Potassium-depleting diuretics (e.g., Furosemide)
Increased risk of hypokalemia.
Antidiabetics (e.g., Insulin, Oral Hypoglycemics)
Corticosteroids can increase blood glucose levels, requiring dose adjustment of antidiabetics.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
May increase corticosteroid levels, enhancing effects and side effects.
Storage
Store at controlled room temperature (20-25°C), protect from freezing and excessive heat. Do not refrigerate. Keep away from light.
Overdose
Acute overdose is rare. Symptoms may include fluid retention, hypertension, and hyperglycemia. Management is primarily supportive and symptomatic. Gradual withdrawal of the drug should be considered under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Corticosteroids are excreted in breast milk; caution advised during lactation. Discuss with a doctor.
