Overview of the medicine
Centradol 100 mg tablet contains Tramadol Hydrochloride, which is a synthetic opioid analgesic used to relieve moderate to moderately severe pain. It acts on the central nervous system to reduce the feeling of pain.
Uses & Indications
Dosage
Adults
The usual starting dose is 50-100 mg every 4-6 hours as needed. Do not exceed 400 mg per day. For Centradol 100 mg, one tablet every 4-6 hours as needed, with a maximum of 4 tablets per day.
Elderly
For patients over 75 years, the maximum dose should not exceed 300 mg per day. Dosage intervals may be extended.
Renal_impairment
In patients with creatinine clearance less than 30 mL/min, the dosing interval should be extended to every 12 hours. A maximum of 200 mg per day should not be exceeded.
How to Take
Centradol 100 mg tablet should be taken orally, with or without food. Swallow the tablet whole; do not crush, chew, or break it.
Mechanism of Action
Tramadol acts as a weak opioid agonist at mu-opioid receptors. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effects by modulating the descending inhibitory pain pathways.
Pharmacokinetics
Onset
Onset of analgesic effect is typically within 1 hour.
Excretion
Excreted primarily by the kidneys (about 90% as metabolites and 10% as unchanged drug).
Half life
Approximately 6.3 hours for tramadol and 7.4 hours for the active metabolite O-desmethyltramadol (M1).
Absorption
Rapidly and almost completely absorbed after oral administration, with an absolute bioavailability of about 75%.
Metabolism
Extensively metabolized in the liver by CYP2D6 and CYP3A4 enzymes, mainly to O-desmethyltramadol (M1, active) and N-desmethyltramadol (M2, inactive).
Side Effects
Contraindications
- •Hypersensitivity to tramadol or any component of the formulation
- •Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs
- •Patients who have taken monoamine oxidase inhibitors (MAOIs) within the last 14 days
- •Severe respiratory depression
- •Uncontrolled epilepsy or during treatment for opioid withdrawal
Drug Interactions
MAOIs
Concomitant use with MAOIs can lead to serotonin syndrome or seizures. Avoid use within 14 days of MAOIs.
Warfarin
May increase INR and risk of bleeding.
SSRIs/SNRIs
Increased risk of serotonin syndrome and seizures.
Carbamazepine
Significantly decreases tramadol plasma concentrations, potentially reducing analgesic efficacy.
CNS Depressants
Additive CNS depressant effects when used with alcohol, sedatives, hypnotics, or other opioids, increasing risk of respiratory depression and profound sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include respiratory depression, somnolence, coma, seizures, constricted pupils, cardiovascular collapse, and death. Management involves maintaining a patent airway, supportive ventilation, and administering naloxone to reverse opioid effects. Seizures can be managed with benzodiazepines.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to the risk of neonatal opioid withdrawal syndrome. Small amounts are excreted in breast milk; therefore, it is generally advised to avoid breastfeeding or use with caution under medical supervision.
