Overview of the medicine
Centradol 75 mg Tablet contains Tramadol Hydrochloride, an opioid analgesic used to relieve moderate to moderately severe pain. It works by changing how your brain and nervous system respond to pain.
Uses & Indications
Dosage
Adults
The usual adult dose is 50-100 mg every 4-6 hours as needed. For Centradol 75 mg, 75 mg every 4-6 hours. Maximum daily dose is 400 mg.
Elderly
For patients over 75 years, maximum dose is 300 mg/day, administered in divided doses. Consider slower titration.
Renal_impairment
For creatinine clearance less than 30 mL/min, the dose should be reduced to 50-100 mg every 12 hours. Maximum daily dose 200 mg. Not recommended for severe impairment.
How to Take
Take orally with or without food. Swallow the tablet whole; do not crush, chew, or split it. Take exactly as prescribed by your doctor.
Mechanism of Action
Tramadol acts as a weak mu-opioid receptor agonist. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effect.

Onset
Analgesia typically begins within 30-60 minutes.
Excretion
Primarily excreted via the kidneys (about 90% as metabolites, 10% as unchanged drug).
Half life
Approximately 5-7 hours for tramadol, 9 hours for the active metabolite O-desmethyltramadol.
Absorption
Rapidly absorbed after oral administration. Absolute bioavailability is approximately 75%.
Metabolism
Extensively metabolized in the liver by CYP2D6 (to active M1 metabolite) and CYP3A4 (to inactive metabolites).
Side Effects
Contraindications
- •Hypersensitivity to tramadol or any component of the formulation.
- •Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs.
- •Severe respiratory depression.
- •In patients with a history of seizures not adequately controlled.
- •Concomitant use with MAO inhibitors or within 14 days of discontinuing them.
Drug Interactions
Warfarin
Increased INR and risk of bleeding.
Carbamazepine
Significantly decreases tramadol plasma concentrations, potentially reducing analgesic effect.
MAO Inhibitors
Increased risk of serotonin syndrome and seizures.
SSRIs, SNRIs, TCAs
Increased risk of serotonin syndrome and seizures.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased risk of respiratory depression, profound sedation, coma, and death.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include respiratory depression, somnolence progressing to stupor or coma, skeletal muscle flaccidity, cold and clammy skin, constricted pupils, and sometimes circulatory collapse, hypotension, and death. Seizures have also been reported. Treatment includes maintaining a patent airway, providing assisted ventilation, and administering naloxone for opioid reversal. Gastric lavage may be considered.
Pregnancy & Lactation
Not recommended during pregnancy unless benefits outweigh risks, due to potential for neonatal opioid withdrawal syndrome. Tramadol is excreted into breast milk; use during lactation should be avoided.
