Description
Overview of the medicine
Cetril (Cetirizine) is a second-generation antihistamine used to relieve symptoms of allergies such as hay fever, conjunctivitis, hives, and other allergic skin reactions. It works by blocking histamine, a natural substance the body produces during an allergic reaction, thereby reducing itching, sneezing, watery eyes, and runny nose.
Uses & Indications
Dosage
Adults
10 mg once daily. Some individuals may benefit from 5 mg twice daily. Maximum dose 10 mg/day.
Elderly
Generally 5 mg once daily, especially with mild renal impairment. Caution is advised.
Renal_impairment
Moderate renal impairment (creatinine clearance 31-50 ml/min): 5 mg once daily. Severe renal impairment (creatinine clearance <31 ml/min): 5 mg every other day.
How to Take
Cetril can be taken with or without food. Swallow the tablet whole with water. For syrup, use the provided measuring spoon/cup.
Mechanism of Action
Cetirizine is a potent and highly selective peripheral H1-receptor antagonist. It prevents the effects of histamine by blocking its binding to H1 receptors, leading to reduced symptoms of allergy. Unlike first-generation antihistamines, it has low affinity for central H1 receptors, which results in less sedative effects.
Pharmacokinetics
Onset
Onset of action typically occurs within 20-60 minutes.
Excretion
Primarily excreted unchanged in the urine (approximately 60%) and feces (approximately 10%).
Half life
Approximately 8-12 hours in adults; prolonged in renal impairment.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached within 1 hour.
Metabolism
Minor metabolism in the liver; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives.
- Patients with severe renal impairment (creatinine clearance less than 10 ml/min).
Drug Interactions
Theophylline
A slight decrease in cetirizine clearance may occur, but no dose adjustment is usually necessary.
Central Nervous System (CNS) Depressants/Alcohol
Concurrent use may enhance CNS depression (e.g., drowsiness, sedation).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, sedation, tachycardia, and urinary retention. In case of overdose, seek immediate medical attention. Management is supportive and symptomatic; gastric lavage may be considered if recent ingestion. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cetirizine is excreted in breast milk; therefore, use with caution in nursing mothers.
