Overview of the medicine
Chlordiazepoxide is a benzodiazepine used to treat anxiety disorders, symptoms of anxiety, acute alcohol withdrawal, and preoperative apprehension and anxiety.
Uses & Indications
Dosage
Adults
Anxiety: 5-25 mg 3-4 times daily. Acute Alcohol Withdrawal: Initial 50-100 mg, then 25-50 mg 3-4 times daily as needed. Max 300 mg/day.
Elderly
Lower doses, e.g., 5 mg 2-4 times daily, due to increased sensitivity and potential for adverse effects.
Renal_impairment
Dose adjustment may be needed; start with lower doses and monitor response.
How to Take
Take orally with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Chlordiazepoxide works by enhancing the effects of gamma-aminobutyric acid (GABA), a neurotransmitter that inhibits neuronal activity in the brain, thereby producing a calming effect.

Side Effects
Contraindications

Drug Interactions
Opioids
Increased risk of respiratory depression, profound sedation, coma, and death.
CYP450 Inhibitors (e.g., cimetidine, fluvoxamine)
May decrease the metabolism of chlordiazepoxide, leading to increased plasma concentrations and enhanced effects.
CNS Depressants (e.g., alcohol, other anxiolytics, antidepressants, antihistamines)
Enhanced CNS depressant effects leading to increased sedation, respiratory depression.

Overdose
Symptoms of overdose include somnolence, confusion, impaired coordination, diminished reflexes, and coma. Management is primarily supportive; flumazenil, a benzodiazepine receptor antagonist, can be used but with caution due to the risk of precipitating withdrawal or seizures.
Pregnancy & Lactation
Pregnancy Category D: There is positive evidence of human fetal risk, but the benefits from use in pregnant women may be acceptable in certain situations despite the risk. Chlordiazepoxide is excreted in breast milk; therefore, use in lactating mothers should be avoided or used with extreme caution.
