Overview of the medicine
Chear 50 mg tablet is a non-steroidal anti-inflammatory drug (NSAID) primarily used for symptomatic relief in the treatment of osteoarthritis, rheumatoid arthritis, and for the management of acute pain. It works by reducing inflammation and pain.
Uses & Indications
Dosage
Adults
For osteoarthritis and rheumatoid arthritis: 50 mg once daily. For acute pain: 50 mg as a single dose, or 50 mg twice daily if needed.
Elderly
Usually no dosage adjustment is required, but caution is advised due to increased risk of adverse effects.
Renal_impairment
Initial dose reduction to 50 mg every other day may be considered for severe impairment (CrCl < 30 mL/min). Not recommended for severe renal disease.
How to Take
Administer orally, with or without food. Taking it with food may reduce gastrointestinal discomfort.
Mechanism of Action
Chear selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, mediators of pain and inflammation. By inhibiting COX-2, it reduces the production of these inflammatory prostaglandins, thereby providing anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Within 1 hour for pain relief, but full anti-inflammatory effects may take several days.
Excretion
Excreted predominantly in the urine (about 27%) and feces (about 57%) as metabolites; less than 1% as unchanged drug.
Half life
Approximately 8-12 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in approximately 2-4 hours after oral administration.
Metabolism
Primarily hepatic via cytochrome P450 (CYP2C9) to inactive metabolites.

Contraindications
- Hypersensitivity to chear or other NSAIDs
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe heart failure
- Third trimester of pregnancy

Not recommended in the third trimester of pregnancy due to risk of premature closure of ductus arteriosus. Use during early pregnancy and lactation only if clearly needed and benefits outweigh risks.
