Description
Overview of the medicine
Cinamycin 600 mg Injection is a broad-spectrum antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
600 mg administered intravenously once daily, or 300 mg twice daily, infused over 30-60 minutes.
Elderly
No specific dose adjustment is typically required based on age alone, but renal function should be monitored.
Renal_impairment
For creatinine clearance < 30 mL/min, the dose should be reduced to 300 mg once daily.
How to Take
Cinamycin 600 mg Injection should be administered slowly as an intravenous infusion over 30-60 minutes. Do not administer as a rapid bolus injection. Dilute in an appropriate IV fluid (e.g., 0.9% Sodium Chloride, 5% Dextrose).
Mechanism of Action
Cinamycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing peptide chain elongation and leading to a bacteriostatic effect against susceptible organisms. At higher concentrations, it may exhibit bactericidal activity.
Pharmacokinetics
Onset
Within 1-2 hours of administration.
Excretion
Mainly excreted through bile and feces, with a small portion via urine.
Half life
Approximately 4-6 hours.
Absorption
Rapid and complete systemic absorption following intravenous administration.
Metabolism
Primarily hepatic metabolism via cytochrome P450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to Cinamycin or any macrolide antibiotic.
- Patients with a history of cholestatic jaundice/hepatic dysfunction associated with prior Cinamycin use.
Drug Interactions
Digoxin
Increased digoxin levels, monitor for toxicity.
Antacids
May reduce absorption (less relevant for IV but still a consideration for concurrent oral meds).
Warfarin
Increased anticoagulant effect, monitor INR.
Theophylline
Increased theophylline levels, monitor for toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Do not freeze. Reconstituted solution should be used immediately.
Overdose
In case of overdose, symptomatic and supportive measures should be initiated. Gastric lavage is not typically useful for injections. Hemodialysis and peritoneal dialysis are generally not effective in removing Cinamycin.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, use with caution during lactation.

Lab Monitoring
- Liver function tests (LFTs) periodically, especially in patients with pre-existing hepatic impairment.
- Renal function tests (RFTs) for patients with renal impairment or on prolonged therapy.
- Complete blood count (CBC) during extended treatment.
Doctor Notes
- Assess renal and hepatic function prior to and during prolonged therapy.
- Monitor for signs and symptoms of C. difficile-associated diarrhea (CDAD) in all patients presenting with diarrhea following antibiotic use.
- Ensure proper reconstitution and slow IV infusion rate to minimize injection site reactions.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if you feel better.
- Report any severe side effects or allergic reactions immediately to your healthcare provider.
- Do not share this medicine with others, even if they have similar symptoms.
Missed Dose Advice
If a dose of Cinamycin injection is missed, it should be administered as soon as possible. However, if it is almost time for the next scheduled dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to compensate.
