Description
Overview of the medicine
Clorocef 125 mg Pediatric Drop is a cephalosporin antibiotic used to treat various bacterial infections in children, such as ear infections, throat infections, lung infections, and skin infections.
Uses & Indications
Dosage
Adults
Not typically used for adults in this specific 'pediatric drop' form. Adult dosage for Cefaclor is generally 250-500 mg every 8 hours.
Elderly
As per adult dosage, with careful consideration for renal function. Dosage adjustment may be necessary.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For CrCl 10-50 mL/min, administer 50% of the usual dose. For CrCl <10 mL/min, administer 25% of the usual dose.
How to Take
Clorocef Pediatric Drop should be administered orally, with or without food. Use the provided dropper for accurate measurement. Shake the suspension well before each use. Complete the full course of treatment as prescribed by the physician.
Mechanism of Action
Cefaclor is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 30-60 minutes.
Excretion
Mainly excreted by the kidneys through glomerular filtration and tubular secretion. Approximately 60-85% of the dose is excreted unchanged in the urine within 8 hours.
Half life
Approximately 0.5-1 hour.
Absorption
Rapidly and almost completely absorbed after oral administration. Food may delay absorption but not significantly reduce the extent.
Metabolism
Limited hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Known hypersensitivity to cefaclor or other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
Increases cefaclor plasma levels and prolongs its half-life by inhibiting renal tubular secretion.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity.
Warfarin and other oral anticoagulants
May increase the anticoagulant effect, requiring close monitoring of prothrombin time (INR).
Storage
Store the unreconstituted powder at room temperature (below 25°C), away from light and moisture. Do not freeze. Store the reconstituted suspension in a refrigerator (2-8°C) and discard any unused portion after 14 days.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management primarily involves gastric lavage if ingestion is recent, and supportive care. Hemodialysis is generally not effective. Contact a poison control center immediately.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use in pregnancy should only be if clearly needed. Cefaclor is excreted in low concentrations in human milk; caution should be exercised when administered to a nursing mother.

- Periodical monitoring of renal function is advised during prolonged therapy.
- Complete blood count (CBC) may be monitored during extended treatment periods.
Doctor Notes
- Always inquire about a history of penicillin allergy before prescribing cefaclor due to potential cross-reactivity.
- Adjust dosage for patients with renal impairment based on creatinine clearance.
- Educate parents/caregivers on the importance of completing the full course of treatment and proper administration using the provided dropper.
Patient Guidelines
- Complete the entire course of the antibiotic as prescribed by your doctor, even if symptoms improve.
- Do not share this medication with others, as it may not be suitable for their condition.
