Description
Overview of the medicine
Clorocef 250 mg capsule contains Cefaclor, a broad-spectrum second-generation cephalosporin antibiotic. It is used to treat various bacterial infections susceptible to Cefaclor.
Uses & Indications
Dosage
Adults
Usually 250 mg every 8 hours. For more severe infections or those caused by less susceptible organisms, doses may be doubled. Maximum dose 4 g/day.
Elderly
No specific dosage adjustment is usually necessary unless there is severe renal impairment.
Renal_impairment
For severe renal impairment (creatinine clearance <10 mL/min), initial dose 250-500 mg followed by 250 mg every 12 hours. Careful clinical and laboratory monitoring is recommended.
How to Take
Clorocef capsules should be swallowed whole with water. Can be taken with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefaclor is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are usually attained within 0.5 to 1 hour.
Excretion
Primarily excreted unchanged by the kidneys, with about 60-85% of the dose recovered in the urine within 8 hours.
Half life
Approximately 0.6 to 1 hour in individuals with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Food may delay absorption but does not affect the total amount absorbed.
Metabolism
Minimal metabolism occurs.
Side Effects
Contraindications
- Hypersensitivity to Cefaclor or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin (due to potential cross-reactivity).
Drug Interactions
Probenecid
Increases plasma concentrations and prolongs the half-life of Cefaclor.
Aminoglycosides
Concurrent use with some cephalosporins has been associated with nephrotoxicity.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect; INR monitoring recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management is symptomatic and supportive, including gastric lavage and activated charcoal if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy should be only if clearly needed. Cefaclor is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.

Clinical Trials
Cefaclor has been extensively studied in various clinical trials demonstrating its efficacy and safety in treating a wide range of bacterial infections. Ongoing research focuses on antibiotic resistance patterns.
Lab Monitoring
- Complete Blood Count (CBC) if used for prolonged periods.
- Renal function tests (e.g., creatinine, BUN) in patients with pre-existing renal impairment or during prolonged high-dose therapy.
Doctor Notes
- Cefaclor is generally well-tolerated. Counsel patients on the importance of completing the full course. Monitor for hypersensitivity reactions, especially in penicillin-allergic patients. Adjust dose in severe renal impairment.
- Consider stool sample testing for CDAD if persistent diarrhea occurs.
Patient Guidelines
- Take the full course of medicine as prescribed by your doctor, even if you feel better.
- Do not share your medicine with others.
- Inform your doctor about any history of allergies, especially to antibiotics.
- Contact your doctor if side effects persist or worsen.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cefaclor may cause dizziness in some patients. Patients should be advised to exercise caution before driving or operating machinery until they know how Cefaclor affects them.
Lifestyle Advice
- Maintain good hydration by drinking plenty of fluids.
- Avoid alcohol consumption, which may worsen gastrointestinal side effects.
- Follow proper hygiene practices to prevent reinfection.
