Overview of the medicine
Clorocef 125 mg Suspension is an antibiotic used to treat various bacterial infections, including those of the respiratory tract, skin and soft tissue, and urinary tract. It belongs to the cephalosporin class of antibiotics.
Uses & Indications
Dosage
Adults
Generally 250-500 mg every 8 hours. Dosage for suspension (125mg/5ml) for adults is less common; often higher strengths are used. Max 4g/day.
Elderly
Similar to adults, with renal adjustment as necessary.
Children
20-40 mg/kg/day in divided doses every 8 hours. Max 1g/day. For 125mg/5ml suspension: Children 1 month to 1 year: 2.5 ml 3 times daily. Children 1-5 years: 5 ml 3 times daily. Children >5 years: 5-10 ml 3 times daily.
Renal_impairment
For creatinine clearance 10-50 mL/min, administer usual dose but increase interval; for <10 mL/min, administer 50% of usual dose with increased interval.
How to Take
Clorocef suspension should be taken orally, with or without food. Shake the bottle well before each use. A measuring spoon or cup should be used for accurate dosing.
Mechanism of Action
Cefaclor inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Excretion
Excreted primarily by the kidneys, with approximately 60-85% of the dose recovered unchanged in the urine within 8 hours.
Half life
Approximately 0.5 to 1 hour.
Absorption
Well absorbed from the gastrointestinal tract, reaching peak plasma concentrations within 0.5-1 hour. Food may delay absorption but not the total amount absorbed.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefaclor or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
May inhibit the renal excretion of cefaclor, leading to increased and prolonged plasma concentrations.
Warfarin and oral anticoagulants
Increased prothrombin time (INR) and potential for bleeding; monitor INR closely.
Aminoglycosides and potent diuretics
Possible increased nephrotoxicity (rare with oral cefaclor).
Storage
Store the dry powder below 30°C in a dry place, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management is primarily supportive; gastric lavage may be considered if ingestion is recent. Hemodialysis is not effective in removing cefaclor.

Pregnancy Category B. Cefaclor should be used during pregnancy only if clearly needed. It is excreted in breast milk in small amounts; use with caution in nursing mothers.
