Description
Overview of the medicine
Comfy-05 mg Tablet contains Zolpidem Tartrate, a non-benzodiazepine hypnotic used for the short-term treatment of insomnia. It helps individuals fall asleep faster.
Uses & Indications
Dosage
Adults
For non-extended release formulations, the recommended initial dose is 5 mg for women and 5 mg or 10 mg for men, taken once daily immediately before bedtime. The total dose should not exceed 10 mg.
Elderly
The recommended initial and maximum dose is 5 mg once daily immediately before bedtime.
Renal_impairment
No dosage adjustment needed. However, use with caution in patients with severe renal impairment.
How to Take
Take orally, once daily immediately before bedtime. Do not take with or immediately after a meal as this may delay the onset of action and reduce its effectiveness. Take on an empty stomach.
Mechanism of Action
Zolpidem is a non-benzodiazepine hypnotic that selectively binds to the benzodiazepine omega-1 receptor (a subtype of GABA-A receptors) on the GABA-A receptor complex. This binding enhances the inhibitory effects of GABA, leading to sedative, hypnotic, anxiolytic, anticonvulsant, and muscle relaxant properties, primarily promoting sleep.
Pharmacokinetics
Onset
Approximately 15-30 minutes
Excretion
Mainly excreted in urine (about 48%) and feces (about 33%) as inactive metabolites.
Half life
Approximately 2.5 hours
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically achieved within 0.5 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP3A4, CYP2C9, CYP1A2, CYP2D6, CYP2C19) oxidation to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to Zolpidem or any inactive ingredients.
- •History of complex sleep behaviors after taking Zolpidem.
- •Severe respiratory insufficiency.
- •Severe hepatic impairment.
Drug Interactions
Alcohol
Potentiates CNS depressant effects, increasing risk of adverse events.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole)
May increase Zolpidem exposure, requiring dose adjustment.
CNS Depressants (e.g., opioids, benzodiazepines, tricyclic antidepressants)
Increased risk of CNS depression, respiratory depression, and profound sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, confusion, ataxia, slurred speech, and potentially coma. Treatment is primarily supportive and symptomatic. Gastric lavage and activated charcoal may be considered if ingestion is recent. Flumazenil may be considered but is not a routine antidote for non-benzodiazepines.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy unless the potential benefit outweighs the potential risk. Excreted in breast milk; advise against use during lactation.
