Description
Overview of the medicine
Vitamin D3 (Cholecalciferol) is a fat-soluble vitamin essential for the absorption of calcium and phosphorus, crucial for bone health and immune function. This capsule provides a high dose for the treatment and prevention of Vitamin D deficiency.
Uses & Indications
Dosage
Adults
Typically 20000 IU once weekly or once every two weeks for severe deficiency, as directed by a physician. Dosage varies based on deficiency severity and patient response.
Elderly
Same as adults, but monitor renal function and calcium levels more closely.
Renal_impairment
Use with caution; monitoring of calcium and phosphate levels is crucial. Dosage may need adjustment under strict medical supervision.
How to Take
Orally with water, preferably with a fatty meal to enhance absorption as it is fat-soluble.
Mechanism of Action
Cholecalciferol is converted in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the active form of Vitamin D. Calcitriol promotes calcium absorption from the gut, regulates serum calcium and phosphate concentrations, and plays a role in bone mineralization.
Pharmacokinetics
Onset
Effects on calcium metabolism are generally seen within hours to days, but sustained changes take weeks.
Excretion
Primarily via bile into feces. Small amounts excreted in urine.
Half life
Parent compound (cholecalciferol) has a half-life of about 24 hours; 25-hydroxycholecalciferol (calcidiol) has a half-life of 2-3 weeks.
Absorption
Readily absorbed from the small intestine in the presence of bile salts, preferably with fatty meals.
Metabolism
Hydroxylated in the liver to 25-hydroxycholecalciferol and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol).
Side Effects
Contraindications
- •Hypercalcaemia (high calcium levels in blood).
- •Hypervitaminosis D (Vitamin D toxicity).
- •Severe renal impairment with hyperphosphataemia.
- •Known hypersensitivity to cholecalciferol or any excipients.
Drug Interactions
Corticosteroids
May reduce Vitamin D effects.
Thiazide diuretics
May increase risk of hypercalcaemia.
Phenytoin, barbiturates
May increase Vitamin D metabolism, reducing its effects.
Cholestyramine, mineral oil, orlistat
May reduce Vitamin D absorption.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypercalcaemia (nausea, vomiting, loss of appetite, constipation, weakness, fatigue, headache, muscle pain, polydipsia, polyuria). Severe cases can lead to renal damage. Management involves discontinuing Vitamin D, restricting calcium intake, and ensuring adequate hydration. In severe cases, corticosteroids or calcitonin may be used. Monitor serum calcium and renal function.
Pregnancy & Lactation
Safe when used in recommended doses during pregnancy and lactation. High doses should be avoided due to potential fetal toxicity. Consult a doctor.
