Description
Overview of the medicine
Dancel-2-Cream is a combination topical medication containing Miconazole Nitrate, an antifungal, and Clobetasol Propionate, a potent corticosteroid. It is used to treat various fungal skin infections that are accompanied by inflammation, itching, or redness.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should not exceed 2 weeks.
Elderly
No specific dosage adjustment required, but use with caution due to thinner skin and potential for increased absorption.
Renal_impairment
No specific dosage adjustment required for topical application.
How to Take
For external use only. Wash hands before and after application. Apply a thin film of cream to the affected skin area and gently rub in. Do not apply to the eyes, mouth, or other mucous membranes. Do not use with occlusive dressings unless directed by a physician.
Mechanism of Action
Miconazole Nitrate inhibits the biosynthesis of ergosterol, a vital component of fungal cell membranes, leading to increased permeability and eventual lysis of the fungal cell. Clobetasol Propionate is a highly potent corticosteroid that exerts anti-inflammatory, antipruritic, and vasoconstrictive actions by inducing phospholipase A2 inhibitory proteins, thereby controlling the synthesis of inflammatory mediators like prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Symptomatic relief can be expected within a few days; complete resolution of infection may take longer.
Excretion
Metabolites of Clobetasol are primarily excreted in the urine and feces.
Half life
Not well-defined for topical application due to minimal systemic absorption; for absorbed Clobetasol, plasma half-life is approximately 3-4 hours.
Absorption
Miconazole Nitrate has minimal systemic absorption when applied topically. Clobetasol Propionate can be absorbed systemically, especially when applied to large areas, broken skin, or under occlusive dressings. Systemic absorption depends on the vehicle, integrity of the epidermal barrier, and use of occlusive dressings.
Metabolism
Absorbed Clobetasol is primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Miconazole, Clobetasol, or any excipients
- Untreated bacterial, viral (e.g., herpes simplex, varicella), or parasitic skin infections
- Acne rosacea, acne vulgaris, perioral dermatitis
- Pruritus without inflammation
- Peri-anal and genital pruritus
Drug Interactions
Other topical corticosteroids
Concurrent use may increase the risk of systemic side effects.
Warfarin (if significant systemic absorption)
Clobetasol may enhance the anticoagulant effect of warfarin.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Topical overdose is unlikely to produce acute life-threatening effects. Prolonged or excessive use may lead to systemic effects of corticosteroids (e.g., Cushing's syndrome) and local side effects (e.g., skin atrophy). Treatment is symptomatic and supportive; discontinue the cream gradually if systemic effects are suspected.
Pregnancy & Lactation
Use with caution during pregnancy and lactation. Systemic absorption of corticosteroids can occur. Consult a physician before use. Avoid prolonged use or application to large areas.

Clinical Trials
Clinical efficacy and safety data support the use of Miconazole and Clobetasol in combination for dermatological fungal infections with inflammation.
Lab Monitoring
- Not routinely required for short-term topical use. For prolonged or extensive use, HPA axis suppression may be monitored (e.g., plasma cortisol levels).
Doctor Notes
- Emphasize limited duration of use (max 2 weeks) to minimize corticosteroid side effects.
- Counsel patients on proper application technique and hygiene.
- Monitor for signs of secondary infection or worsening condition, which may require discontinuation.
Patient Guidelines
- Use only as directed by your doctor. Do not exceed the prescribed duration of treatment.
- Avoid applying to the face, groin, or armpits unless specifically instructed by your doctor.
- Keep out of reach of children. Store at room temperature, away from heat and direct light.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not apply a double dose to make up for a missed one.
