Description
Overview of the medicine
Dexamethasone is a potent synthetic corticosteroid with anti-inflammatory and immunosuppressant properties. It is used to treat a wide range of conditions, including allergic disorders, skin conditions, ulcerative colitis, arthritis, lupus, psoriasis, and breathing disorders.
Uses & Indications
Dosage
Adults
Initial dose 0.5-9 mg daily, divided into 2-4 doses, adjusted according to response. For severe cases, up to 10-15 mg/day.
Elderly
Similar to adults, but with caution due to increased risk of side effects and comorbidities. Lower doses may be appropriate.
Renal_impairment
No specific dosage adjustment required; however, monitor carefully.
How to Take
Oral. Take with food or milk to minimize gastrointestinal upset. Swallow whole with water.
Mechanism of Action
Dexamethasone binds to glucocorticoid receptors in the cytoplasm, forming a complex that translocates to the nucleus. This complex then modulates gene expression, leading to the synthesis of anti-inflammatory proteins (e.g., lipocortin-1) and inhibition of pro-inflammatory mediators (e.g., cytokines, prostaglandins, leukotrienes).
Pharmacokinetics
Onset
Within hours to days depending on condition.
Excretion
Primarily renal (urine) as inactive metabolites.
Half life
Plasma half-life 3-4 hours; biological half-life 18-54 hours.
Absorption
Rapidly and almost completely absorbed from the GI tract. Peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily hepatic, mainly by 6β-hydroxylation.
