Description
Overview of the medicine
Dexcor (Dexamethasone) is a synthetic corticosteroid drug. It is used for its potent anti-inflammatory and immunosuppressive effects in various conditions.
Uses & Indications
Dosage
Adults
Initial dose varies from 0.5 mg to 9 mg daily, depending on the disease being treated. Maintenance dose should be individualized.
Elderly
Elderly patients should be treated with caution, starting with the lowest effective dose due to increased risk of side effects.
Renal_impairment
No specific dose adjustment is usually required for renal impairment, but caution is advised in severe cases.
How to Take
Oral tablets should be taken with food or milk to minimize gastrointestinal irritation. Injections are administered intravenously or intramuscularly.
Mechanism of Action
Dexamethasone binds to intracellular glucocorticoid receptors, forming a complex that translocates to the nucleus. This complex then modulates gene expression, leading to the synthesis of anti-inflammatory proteins and inhibition of pro-inflammatory mediators (e.g., prostaglandins, leukotrienes).
Pharmacokinetics
Onset
Onset of anti-inflammatory action occurs within hours; full therapeutic effects may take several days.
Excretion
Mainly excreted in urine as inactive metabolites.
Half life
Plasma half-life: 3-4.5 hours; Biological half-life: 36-54 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by CYP3A4 enzymes.
Side Effects
Contraindications
- •Systemic fungal infections
- •Known hypersensitivity to Dexamethasone or any component of the formulation
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration and bleeding.
Anticoagulants e.g., Warfarin
Altered anticoagulant effects; monitor INR.
Antidiabetics e.g., Insulin, Metformin
May increase blood glucose levels, requiring dose adjustment of antidiabetics.
Diuretics e.g., Thiazides, Loop diuretics
Increased risk of hypokalemia.
CYP3A4 Inducers e.g., Phenobarbital, Phenytoin, Rifampicin
May decrease Dexamethasone plasma levels.
CYP3A4 Inhibitors e.g., Ketoconazole, Macrolide antibiotics
May increase Dexamethasone plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare and usually not life-threatening. Chronic overdose can lead to symptoms of Cushing's syndrome. Treatment is symptomatic and supportive; gradual withdrawal if chronic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Dexamethasone is excreted in breast milk; advise caution during lactation.
