Description
Overview of the medicine
Dextropac-10-injection is a sterile solution of Dextrose (glucose) 10% w/v in water for injection, administered intravenously. It is primarily used as a source of calories and fluid replacement, and to treat hypoglycemia.
Uses & Indications
Dosage
Adults
Dosage varies depending on patient's age, weight, clinical condition, and fluid/electrolyte balance. Typically, 500-1000 mL of 10% dextrose solution per day, administered slowly intravenously. Max infusion rate should not exceed 0.5 g/kg/hour.
Elderly
Same as adults, but caution should be exercised due to potential age-related renal or cardiac impairment; close monitoring of fluid and electrolyte status is advised.
Renal_impairment
Use with caution and monitor fluid and electrolyte balance closely. Lower rates of infusion may be necessary to prevent fluid overload or electrolyte disturbances.
How to Take
Administered by slow intravenous infusion through a peripheral or central vein. The rate of administration should be carefully controlled by a healthcare professional based on the patient's metabolic needs and glucose tolerance.
Mechanism of Action
Dextrose provides readily available carbohydrates, which are metabolized to produce energy, carbon dioxide, and water. It helps in maintaining fluid and electrolyte balance and prevents ketosis.
Pharmacokinetics
Onset
Immediate onset of action upon infusion.
Excretion
Water and carbon dioxide are excreted via kidneys and lungs respectively. Unmetabolized dextrose is excreted renally if plasma levels exceed renal threshold.
Half life
Rapidly metabolized; no specific plasma half-life for dextrose itself as it's continuously used by the body.
Absorption
Administered intravenously, hence 100% bioavailable.
Metabolism
Metabolized to carbon dioxide and water through glycolysis and the Krebs cycle, primarily in tissues.
Side Effects
Contraindications
- •Known hypersensitivity to dextrose or corn products (if corn-derived).
- •Anuria (absence of urine production).
- •Diabetic coma (unless blood sugar is very low and insulin is administered concurrently).
- •Intracranial or intraspinal hemorrhage (due to potential for increased intracranial pressure).
- •Patients with known fructose-sorbitol intolerance (depending on source of dextrose).
Drug Interactions
Diuretics
May alter electrolyte balance, requiring close monitoring when co-administered with dextrose solutions.
Corticosteroids
May increase blood glucose levels, potentially requiring adjustment of dextrose dose or insulin.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children. Use immediately after opening.
Overdose
Overdose may lead to severe hyperglycemia, glycosuria, dehydration, electrolyte disturbances (e.g., hypokalemia, hypophosphatemia), and osmotic diuresis. Treatment involves discontinuing infusion, administering insulin if severe hyperglycemia, and correcting fluid/electrolyte imbalances.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation, as dextrose is a natural metabolic substrate. However, careful monitoring of maternal and fetal/infant blood glucose and electrolytes is recommended.
