Description
Overview of the medicine
Dextrose 5% Injection is an intravenous solution used to provide fluid, carbohydrate, and calories to patients when oral intake is not possible or insufficient. It helps in maintaining hydration and serves as a caloric source.
Uses & Indications
Dosage
Adults
Dosage is individualized based on patient's age, weight, clinical condition, and fluid/caloric needs. Typically, 500 mL to 1000 mL per 24 hours via slow intravenous infusion, at a rate not exceeding 0.5 g/kg/hour (or 10 mL/kg/hour of 5% Dextrose).
Elderly
Similar to adults, but with caution regarding fluid overload and electrolyte imbalances, especially in patients with cardiac or renal impairment.
Renal_impairment
Use with caution and monitor fluid and electrolyte balance closely. Lower infusion rates may be required to prevent fluid overload.
How to Take
Administered by slow intravenous infusion. Do not administer intramuscularly or subcutaneously. Ensure proper aseptic technique during administration.
Mechanism of Action
Provides readily available glucose (dextrose) which is metabolized by cells for energy, thereby supplying calories and maintaining blood glucose levels. It also helps to maintain fluid balance and can be used as a vehicle for other compatible medications.
Pharmacokinetics
Onset
Immediate
Excretion
Water and CO2 are excreted through kidneys and lungs, respectively.
Half life
Depends on the rate of metabolism and utilization by the body, typically rapidly cleared.
Absorption
Administered intravenously, hence 100% bioavailable immediately.
Metabolism
Metabolized to carbon dioxide and water via the glycolytic pathway and Krebs cycle, producing energy.
Side Effects
Contraindications
- •Known allergy to corn or corn products
- •Anuria
- •Diabetic coma (unless treating hypoglycemia)
- •Intracranial or intraspinal hemorrhage (unless rapid volume expansion is critical)
Drug Interactions
Insulin
Dextrose infusion may reduce insulin requirements in diabetic patients. Close monitoring is needed.
Corticosteroids
May increase blood glucose levels, requiring adjustment of dextrose infusion rate.
Storage
Store at room temperature (15-30°C). Do not freeze. Protect from excessive heat and direct sunlight.
Overdose
Overdose may lead to fluid overload, electrolyte disturbances (especially hyponatremia), hyperglycemia, and osmotic diuresis. Management involves discontinuing the infusion, administering diuretics if needed, and correcting fluid and electrolyte imbalances.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation when medically indicated. Monitor fluid and electrolyte balance closely in the mother.
