Overview of the medicine
Dotfix is a broad-spectrum antibiotic from the cephalosporin class, primarily used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
For most infections: 200 mg once daily or 100 mg twice daily for 7-14 days, depending on the severity and type of infection. For uncomplicated gonorrhea: 400 mg as a single oral dose.
Elderly
Same as adult dose, but careful monitoring of renal function is recommended due to potential age-related decline in kidney function.
Renal_impairment
Dose adjustment is necessary for patients with creatinine clearance less than 60 mL/min. For creatinine clearance 21-60 mL/min, 100 mg once daily. For creatinine clearance < 20 mL/min, 50 mg once daily.
How to Take
Dotfix can be taken with or without food. It is important to complete the full course of medication as prescribed by the doctor, even if symptoms improve, to prevent the development of antibiotic resistance.
Mechanism of Action
Dotfix acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which interferes with the transpeptidation process vital for peptidoglycan synthesis, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
1-2 hours for noticeable therapeutic effect.
Excretion
Primarily renal excretion, with approximately 50% of an absorbed dose excreted unchanged in the urine within 24 hours. Some biliary excretion also occurs.
Half life
3-4 hours, which allows for once or twice daily dosing.
Absorption
Well absorbed orally; approximately 40-50% bioavailability. Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Metabolism
Not significantly metabolized by the liver; most of the drug is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cephalosporins or any component of the formulation.
- •Patients with a history of anaphylaxis to penicillin should not take Dotfix due to potential cross-reactivity.
Drug Interactions
Nifedipine
May increase cefixime absorption, leading to higher plasma concentrations.
Probenecid
May decrease the renal excretion of Dotfix, leading to increased and prolonged plasma concentrations.
Carbamazepine
Concomitant administration has been shown to result in increased carbamazepine plasma levels. Therapeutic drug monitoring for carbamazepine is recommended.
Oral anticoagulants (e.g., Warfarin)
Concomitant use may increase the anticoagulant effect. Close monitoring of INR and dose adjustment of the anticoagulant may be necessary.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children. The oral suspension should be reconstituted as directed and used within 7 days.
Overdose
In case of overdose, no specific antidote is available. Gastric lavage may be indicated in the early stages. Treatment should be supportive and symptomatic. Hemodialysis or peritoneal dialysis is not substantially effective in removing Dotfix from the body.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not demonstrated a fetal risk, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Small amounts of Dotfix may be excreted in breast milk; use with caution in nursing mothers.
