Overview of the medicine
Ecox (Etoricoxib) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gout.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30-60 mg once daily. Rheumatoid Arthritis/Ankylosing Spondylitis: 60-90 mg once daily. Acute Gouty Arthritis: 120 mg once daily (maximum 8 days). Acute Pain/Dysmenorrhea: 60-120 mg once daily.
Elderly
No specific dose adjustment is generally required, but caution is advised due to increased risk of adverse effects.
Renal_impairment
Moderate renal impairment (CrCl 30-80 mL/min): initiate with 30 mg once daily. Severe renal impairment (CrCl <30 mL/min): not recommended.
How to Take
Ecox tablets can be taken with or without food. For rapid onset of action, it is recommended to take it without food.
Mechanism of Action
Etoricoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in pain, inflammation, and fever.
Pharmacokinetics
Onset
Pain relief typically occurs within 24 minutes for acute pain.
Excretion
Eliminated predominantly by the kidneys (approximately 70% as metabolites) and via feces (approximately 20% as metabolites).
Half life
Approximately 22 hours.
Absorption
Well absorbed orally, with an absolute bioavailability of approximately 100%. Peak plasma concentrations (Tmax) are reached in about 1 hour.
Metabolism
Extensively metabolized, primarily by cytochrome P450 enzymes (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component of the formulation.
- •Active peptic ulceration or gastrointestinal bleeding.
- •Severe hepatic or renal impairment.
- •Congestive heart failure (NYHA Class II-IV).
- •Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease.
- •Uncontrolled hypertension.
Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
Diuretics
Reduced natriuretic effect.
Methotrexate
Increased plasma methotrexate levels.
ACE Inhibitors/ARBs
May reduce antihypertensive effect and increase risk of renal impairment.
Oral Contraceptives
Increased plasma concentrations of ethinyl estradiol.
Ciclosporin/Tacrolimus
Increased nephrotoxicity.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive care is recommended. There is no specific antidote. Dialysis is unlikely to be effective in removing etoricoxib due to high protein binding.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester due to potential fetal harm. Avoid during lactation as it is unknown if etoricoxib is excreted in human milk.
