Description
Overview of the medicine
Emlozin (Amlodipine) is a calcium channel blocker used to treat high blood pressure (hypertension) and certain types of chest pain (angina). It works by relaxing blood vessels so blood can flow more easily, and by reducing the workload on the heart.
Uses & Indications
Dosage
Adults
For hypertension and angina: Initial dose 5 mg once daily. May be increased to a maximum of 10 mg once daily.
Elderly
Consider starting with a lower dose of 2.5 mg once daily, especially if frail or with hepatic impairment.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with a glass of water. It is advisable to take the medicine at the same time each day.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. It produces vasodilation by acting directly on vascular smooth muscle, leading to a reduction in peripheral vascular resistance and blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect begins gradually over 6-12 hours.
Excretion
Primarily excreted in the urine as inactive metabolites (60% of administered dose) and 10% as unchanged drug.
Half life
Approximately 30-50 hours.
Absorption
Well absorbed after oral administration, with peak plasma concentrations reached in 6-12 hours. Bioavailability is 60-64%.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approximately 90%).
Side Effects
Contraindications
- •Hypersensitivity to Amlodipine or other dihydropyridine derivatives
- •Severe hypotension
- •Cardiogenic shock
- •Obstructive outflow tract disease (e.g., severe aortic stenosis)
- •Hemodynamically unstable heart failure after acute myocardial infarction
Drug Interactions
Simvastatin
Coadministration with Amlodipine may increase systemic exposure to simvastatin. Limit the dose of simvastatin to 20 mg daily.
Clarithromycin
Increased risk of hypotension and acute kidney injury.
CYP3A4 inducers (e.g., Rifampicin)
May decrease plasma concentrations of Amlodipine, reducing its effect.
CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May increase plasma concentrations of Amlodipine, leading to an increased risk of hypotension.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves active cardiac and respiratory monitoring, frequent blood pressure measurements, and supportive measures such as elevation of limbs, judicious administration of fluids, and vasopressors if necessary.
Pregnancy & Lactation
Pregnancy: Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lactation: Amlodipine is excreted in human milk. Consult doctor before use during lactation.
