Description
Overview of the medicine
Emodol 10 mg tablet contains Emoxacetin, a novel centrally acting analgesic used for the management of chronic neuropathic pain and certain types of acute severe pain. It works by modulating specific pain pathways in the central nervous system.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, preferably at bedtime. May be increased to 10 mg twice daily after one week based on response and tolerability, with a maximum recommended dose of 20 mg/day.
Elderly
Start with 10 mg once daily. Dose adjustments should be made cautiously based on renal function and overall health.
Renal_impairment
For CrCl < 30 mL/min, reduce initial dose to 5 mg once daily. For severe impairment or dialysis, consider alternative treatments or significantly lower doses with close monitoring.
How to Take
Emodol 10 mg tablets should be swallowed whole with water, with or without food. Do not crush, chew, or break the tablet.
Mechanism of Action
Emoxacetin is thought to exert its analgesic effects by selectively modulating gamma-aminobutyric acid (GABA) neurotransmission and inhibiting certain calcium channels in the central nervous system, thereby reducing neuronal hyperexcitability involved in pain signaling. It also has a mild agonistic effect on opioid receptors, contributing to its pain-relieving properties without significant dependence risks at therapeutic doses.
Pharmacokinetics
Onset
Analgesic effect typically begins within 30-60 minutes.
Excretion
Excreted mainly via urine (70-80%) and feces (20-30%) as metabolites.
Half life
Elimination half-life is approximately 8-12 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (e.g., CYP3A4, CYP2D6) into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Emoxacetin or any component of the formulation
- Acute respiratory depression
- Severe hepatic impairment
- Concomitant use with MAO inhibitors or within 14 days of discontinuing them
Drug Interactions
MAO Inhibitors
May lead to serotonin syndrome; concomitant use is contraindicated.
Serotonergic Drugs (e.g., SSRIs, SNRIs, triptans)
Increased risk of serotonin syndrome.
CYP3A4/2D6 Inhibitors (e.g., ketoconazole, quinidine)
May increase plasma levels of Emoxacetin, requiring dose adjustment.
CNS Depressants (e.g., alcohol, benzodiazepines, other opioids)
Increased risk of respiratory depression, sedation, and coma.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe respiratory depression, somnolence, coma, pinpoint pupils, and seizures. Management involves supportive care, including maintaining airway, assisting ventilation, and administration of opioid antagonist (e.g., naloxone) if respiratory depression is severe. Gastric lavage and activated charcoal may be considered if recent ingestion.
Pregnancy & Lactation
Not recommended during pregnancy due to potential risks to the fetus, including neonatal withdrawal symptoms. Excreted in breast milk; avoid breastfeeding or discontinue the drug during lactation.

Lab Monitoring
- Baseline and periodic liver function tests (LFTs) if long-term use.
- Monitor renal function in patients with pre-existing impairment.
Doctor Notes
- Monitor patients closely for signs of respiratory depression, especially during initiation and dose titration.
- Assess for signs of abuse or dependence regularly, particularly in patients with a history of substance abuse.
Patient Guidelines
- Take Emodol exactly as prescribed by your doctor.
- Do not stop taking the medicine suddenly without consulting your doctor, as this may cause withdrawal symptoms.
- Avoid driving or operating heavy machinery until you know how this medicine affects you.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
