Amoxicillin is a penicillin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
General dosage: 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the severity and type of infection. For this 250 mg capsule: 250 mg every 8 hours for mild-moderate infections.
Elderly
No specific dosage adjustment needed for elderly patients with normal renal function. Adjust dosage based on renal impairment.
Renal_impairment
Dosage reduction is required. For CrCl 10-30 mL/min, 250 mg every 12 hours; for CrCl <10 mL/min, 250 mg every 24 hours.
How to Take
Take orally with a glass of water, with or without food. Complete the full prescribed course of treatment, even if symptoms improve.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis, leading to bacterial lysis and death.
Pharmacokinetics
Onset
Rapid (within 1-2 hours after oral administration).
Excretion
Primarily excreted unchanged in the urine by tubular secretion and glomerular filtration. Renal excretion accounts for 60-80% of the dose within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, stable in the presence of gastric acid. Peak plasma concentrations achieved in 1-2 hours.
Metabolism
Partially metabolized (10-25% of dose) to inactive penicilloic acid.
Side Effects
Contraindications
- •Hypersensitivity to amoxicillin, other penicillins, or any component of the formulation.
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any beta-lactam antibacterial agent (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Decreases renal tubular secretion of amoxicillin, resulting in increased and prolonged blood levels of amoxicillin.
Allopurinol
Increased incidence of rash, particularly in patients with hyperuricemia.
Methotrexate
May decrease renal tubular secretion of methotrexate, leading to increased methotrexate toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives (rarely reported).
Warfarin and other oral anticoagulants
Increased prothrombin time (INR) has been reported; monitor coagulation parameters.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balances. In severe cases, renal failure may occur. Management involves symptomatic and supportive measures. Hemodialysis can be used to remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B: Studies in animals have shown no evidence of harm to the fetus. Amoxicillin is excreted in small amounts in breast milk; generally considered safe for use during pregnancy and lactation, but caution is advised.
