Description
Overview of the medicine
Erybac 125 mg Suspension is an antibiotic from the macrolide class, used to treat various bacterial infections, especially in patients who are allergic to penicillin. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Adult dosage typically ranges from 250 mg to 500 mg every 6 hours, or 500 mg to 1 g every 12 hours, depending on the infection severity. This 125 mg suspension is primarily for pediatric use.
Elderly
Dosage adjustments are generally not necessary in elderly patients unless there is severe hepatic impairment.
Renal_impairment
No dosage adjustment is usually required for mild to moderate renal impairment. For severe impairment (CrCl <10 ml/min), a reduction to 50-75% of the usual dose may be considered.
How to Take
Oral administration. Shake the suspension well before each use. Can be taken with or without food, but taking it with food may reduce gastrointestinal upset. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Erythromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis without affecting nucleic acid synthesis. This leads to a bacteriostatic effect, though it can be bactericidal at high concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Mainly via bile and feces, with a small amount excreted in urine.
Half life
Approximately 1.5 to 2 hours.
Absorption
Well absorbed from the gastrointestinal tract, though absorption can be variable and affected by food. Ester forms (e.g., erythromycin estolate) are better absorbed.
Metabolism
Primarily hepatic, via CYP3A4 enzymes, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to erythromycin or any macrolide antibiotic
- Co-administration with terfenadine, astemizole, cisapride, pimozide, or colchicine (due to risk of cardiac arrhythmias or severe toxicity)
- Pre-existing hepatic disease
Drug Interactions
Warfarin
Potentiation of anticoagulant effect, leading to increased bleeding risk.
Colchicine
Increased exposure to colchicine, leading to severe toxicity.
Theophylline
Increased serum concentration of theophylline, leading to toxicity.
Statins (e.g., simvastatin, lovastatin)
Increased risk of myopathy/rhabdomyolysis due to elevated statin levels.
Benzodiazepines (e.g., midazolam, triazolam)
Increased sedative effect.
Terfenadine, Astemizole, Cisapride, Pimozide
Increased risk of QT prolongation and serious cardiac arrhythmias.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard unused portion after the specified period (e.g., 7 or 14 days).
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and abdominal discomfort. Acute pancreatitis and reversible hearing loss may also occur. Treatment is supportive, focusing on gastric lavage and symptomatic management. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe, but use only if clearly needed. Erythromycin is excreted in breast milk; caution is advised, but usually compatible with breastfeeding with monitoring for infant side effects.
