Description
Overview of the medicine
Erybac 250 mg Tablet is a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin infections, and some sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250 mg to 500 mg every 6-12 hours, depending on the severity of the infection. Maximum dose usually 4g/day.
Elderly
Similar to adult dosage, but caution with liver/renal impairment.
Renal_impairment
No dose adjustment usually required for mild to moderate impairment. For severe renal impairment, dose reduction may be considered (e.g., 250 mg every 8-12 hours).
How to Take
Take the tablet orally, preferably on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption of erythromycin base/stearate. However, it can be taken with food if gastrointestinal upset occurs. Swallow the tablet whole with a glass of water, do not chew or crush. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Erythromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing translocation of peptide chains. This action is primarily bacteriostatic, but can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Within 1-2 hours.
Excretion
Mainly via bile, with a small portion excreted in urine.
Half life
Approximately 1.5-2 hours.
Absorption
Well absorbed orally, but absorption can vary and is influenced by food (especially for erythromycin base). Peak plasma concentrations are reached in 1-4 hours.
Metabolism
Primarily hepatic, via CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to erythromycin or any other macrolide antibiotic
- •Pre-existing liver disease
- •Concomitant use with terfenadine, astemizole, cisapride, pimozide (due to risk of QT prolongation and cardiac arrhythmias)
- •Concomitant use with statins metabolized by CYP3A4 (e.g., simvastatin, lovastatin) due to increased risk of myopathy/rhabdomyolysis
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased anticoagulant effect, risk of bleeding.
Theophylline
Increased theophylline levels, risk of toxicity.
Carbamazepine
Increased carbamazepine levels, risk of toxicity.
QT prolonging drugs
Increased risk of fatal arrhythmias.
Statins (simvastatin, lovastatin)
Increased risk of myopathy and rhabdomyolysis.
Ergot alkaloids (e.g., ergotamine)
Acute ergot toxicity.
Benzodiazepines (e.g., midazolam, triazolam)
